摘　　要：目的 对比经锁骨下静脉、彩超引导下腋静脉穿刺完全式静脉输液港植入的临床价值。方法 回顾性分析2021年7月至2022年3月在江西省肿瘤医院乳腺外科接受静脉输液港植入的268例患者(A组锁骨下静脉入路216例,B组腋静脉入路52例)的临床资料,记录手术相关信息,包括患者一般资料、手术方式、围手术期及远期并发症情况。结果 A组成功率95.37%、B组成功率98.08%,差异无统计学意义(P>0.05)。A组穿刺次数>2次患者占比高于B组患者(16.67%vs 5.77%,P<0.05);A组手术时间(min)低于B组(23.95±8.83 vs 27.73±5.30,P<0.05)。A组患者夹闭综合征发生率高于B组患者(6.94%vs 0%,P<0.05);2组囊袋血肿、损伤动脉、气胸、切口感染、导管异位、港体翻转及外露、血栓形成等发生率差异无统计学意义(P>0.05)。结论 相较于锁骨下静脉穿刺入路,彩超引导下腋静脉穿刺入路更安全,并发症更低,值得推广。Objective To compare the clinical value of complete vein infusion port implantation through subclavian vein and axillary vein puncture under color Doppler ultrasound guidance.Methods A retrospective analysis of 268 patients treated with infusion port implantation,216 via subclavian vein in Group A and 52 via axillary vein in Group B,record the information about the operation,including the general information of the patients,the operation methods,the perioperative period and the long-term complications.Results The success rate was 95.37%in group A and 98.08%in group B(P>0.05);The incidence of pinch-off syndrome in group A was higher than that in Group B(6.94%vs 0%,P<0.05);The ratio of patients with punctures>2 times in group A was higher than that in Group B(16.67%vs 5.77%,P<0.05);The operative time(min)in Group A was lower than that in Group B(23.95±8.83 vs 27.73±5.30,P<0.05),There was no significant difference in the incidence of pocket hematoma,injured artery,pneumothorax,incision infection,catheter ectopic,port body overturn and exposure,thrombosis between the 2 groups(P>0.05).Conclusion Compared with subclavian vein puncture,axillary vein puncture under color ultrasound guidance is safer and has lower complications,which is worth popularizing.
