腹腔镜下子宫动脉阻断术联合肌瘤切除术治疗子宫肌瘤的有效性及并发症发生率

摘　　要：目的分析腹腔镜下子宫动脉阻断术联合肌瘤切除术治疗子宫肌瘤的有效性及并发症发生率。方法2019年1月至2022年1月采集样本数据,102例子宫肌瘤患者纳入研究,随机分为2组,对照组行腹腔镜下肌瘤切除术治疗,观察组联合子宫动脉阻断术治疗,对比分析2组并发症发生情况、手术情况、内分泌激素水平及卵巢功能、氧化损伤指标、术后生育及复发情况。结果与对照组相比,观察组并发症发生率较低(P<0.05);观察组排气时间、开始下床活动时间较短,术中总出血量较少,(P<0.05)。术前、术后2组E_(2)、LH、FSH及AFC对比差异无统计学意义(P>0.05)。术前、术后2组Vmax、Vmin及RI对比差异无统计学意义(P>0.05)。术前、术后3 d,2组MYO、IMA水平对比差异无统计学意义(P>0.05)。与对照组相比,观察组妊娠率较高,流产率及复发率较低(P<0.05)。结论腹腔镜下子宫动脉阻断术联合肌瘤切除术治疗子宫肌瘤可提高有效性,减少并发症发生。Objective To analyze the effectiveness and complication rate of laparoscopic uterine artery occlusion combined with myomectomy in the treatment of uterine fibroids.Methods 102 patients with uterine fibroids were included and randomly divided into 2 groups.The control group was treated with laparoscopic myomectomy,and the observation group was treated with uterine artery occlusion.The incidence of complications,operation conditions,endocrine hormone levels,ovarian function,oxidative damage indicators,postoperative fertility and recurrence were compared and analyzed between the 2 groups.Results Compared with the control group,the incidence of complications in the observation group was lower,(P<0.05).In the observation group,the exhaust time and the time to get out of bed were shorter,and the total blood loss during the operation was less,(P<0.05).The difference of E_(2),LH,FSH and AFC between the 2 groups before operation and after operation had no statistical significance(P>0.05).The difference of Vmax,Vmin and RI between the 2 groups before operation and after operation had no statistical significance(P>0.05).Before operation and 3 days after operation,the difference in MYO and IMA levels between the 2 groups had no statistical significance(P>0.05).The pregnancy rate in the observation group was higher,and the miscarriage rate and recurrence rate were lower,P<0.05.Conclusion Laparoscopic uterine artery occlusion combined with myomectomy can improve the effectiveness and reduce the incidence of complications in the treatment of uterine fibroids.
