摘 要:目的:分析佳木斯地区CRE的主动筛查在NICU患儿感染防控与治疗中的作用。方法:对2019年10月1日至2020年9月30日收治入本院在NICU住院的患儿,采集全血、脐分泌物、咽拭子、直肠拭子等标本,进行细菌显色培养,检出病原菌进行药敏实验、β-内酰胺酶检测、检出的CRE菌株进行PCR技术及凝胶电泳分析等方法进行检测,应用统计软件SPSS26.0进行数据分析(χ^(2)检验)。结果:经统计学分析发现:日龄≤2d和8~28d细菌分离率差异同样有显著性(χ^(2)=20.86,P<0.002);日龄≤2d和3~7d细菌分离率差异有显著性(χ^(2)=9.54,P<0.002);日龄3~7d及8~28d新生儿的细菌分离率差异无显著性(χ^(2)=0.148,P>0.05)。结论:3d以上的新生儿,特别是羊水吸入、发热新生儿等,可以作为主动筛查对象;本地区新生儿CRE的主要耐药机制是产KPC酶。Objective:To analyze the role of active screening for carbapenem-resistant enterobacteriaceae(CRE)in the prevention and treatment of infections in neonatal intensive care unit(NICU)patients in Jiamusi area.Methods:From October 1,2019,to September 30,2020,samples including whole blood,umbilical secretion,pharyngeal swabs,rectal swabs,etc.,were collected from hospitalized infants in the NICU.Bacterial cultures were performed,and the isolated pathogens were subjected to antimicrobial susceptibility testing,β-lactamase detection,PCR analysis,and gel electrophoresis for CRE strains.Statistical analysis was conducted using SPSS 26 software(χ^(2) test).Results:Statistical analysis revealed significant differences in the isolation rates of bacteria between age groups≤2 days and 8~28 days(χ^(2)=20.86,P<0.002)and between age groups≤2 days and 3~7 days(χ^(2)=9.54,P<0.002).However,there was no significant difference in the bacterial isolation rate between age groups 3~7 days and 8~28 days(χ^(2)=0.148,P>0.05).Conclusion:Infants over 3 days old,especially those with meconium aspiration syndrome or fever,can be actively screened.The main resistance mechanism of CRE in our region is the production of KPC enzymes.
