414例肿瘤患者贫血发生的相关因素分析

摘　　要：目的:研究肿瘤患者贫血的发生率和分布情况,并分析肿瘤类型、肿瘤分期、肿瘤转移、年龄及性别与肿瘤相关性贫血发生率和对患者血红蛋白水平的影响。方法:对2019年12月至2021年07月414例我院收治的癌症患者的临床资料进行回顾性分析,观察不同肿瘤类型患者贫血的发生率和分布情况,对肿瘤类型、肿瘤分期、肿瘤是否转移、年龄、性别与肿瘤贫血发生的相关性进行分析,比较不同肿瘤类型、肿瘤分期、年龄、肿瘤转移性的患者血红蛋白水平。结果:414例患者中贫血152例,贫血发生率为36.71%。不同肿瘤类型患者贫血发生率不同,其中淋巴瘤(45.00%)和妇科肿瘤(44.44%)发生率最高,胃肠道肿瘤次之(42.48%),头颈部肿瘤最低(20.00%);肿瘤类型(P=0.002)、临床分期(P=0.021)、年龄(P=0.020)与肿瘤相关贫血的发生有关,肿瘤是否转移和贫血发生无关(P=0.097);男性肿瘤贫血患者更容易发生轻度贫血,女性则更容易发生中度贫血(P=0.029)。结论:淋巴瘤、妇科肿瘤的患者更容易发生贫血,年龄越大、肿瘤分期越高的患者更容易发生贫血,男性肿瘤贫血患者以轻度贫血为主,女性肿瘤贫血患者则以中度贫血为主。Objective:To investigate the incidence and distribution of cancer-related anemia(CRA),and analyze the influence of tumor type,tumor stage,tumor metastasis,age and gender on the level of cancer-related anemia and hemoglobin level.Methods:414 cancer patients admitted to our hospital from December 2019 to July 2021 were selected.The incidence and distribution of CRA with different tumor types were observed.The correlation between tumor type,tumor stage,tumor metastasis,age,gender and CRA was analyzed.The hemoglobin level of patients with different tumor type,tumor stage,age and tumor metastasis was compared.Results:Among 414 patients,152 were anaemic,and the incidence of CRA was 36.71%.The CRA incidence in patients with different tumor types is different,with the highest incidence of lymphoma(45.00%)and gynecological tumors(44.44%),followed by gastrointestinal tumors(42.48%),and the lowest incidence of head and neck tumors(20.00%).Tumor type(P=0.002),clinical stage(P=0.021)and age(P=0.020)were related to the occurrence of CRA,the tumor metastasis was not related(P=0.097).Men with tumor anemia were more prone to mild anemia,while women were more prone to moderate anemia(P=0.029).Conclusion:Patients with lymphoma and gynecological tumors,older age and higher grade tumor stage are more likely to develop CRA.Male tumor anemia patients are mainly mild anemia,while female tumor anemia patients are mainly moderate anemia.
