摘 要:目的 探讨miR-29a、miR-448在人结肠癌组织中的表达水平及与临床预后的关系。方法 选取2019年1月—12月本院收治的结肠癌患者67例为研究对象。收集结肠癌患者临床资料,取手术切除的结肠癌组织及癌旁组织,检测miR-29a、miR-448表达水平。随访6个月~36个月(第36个月为终点时间),剔除失访5例,根据预后情况将结肠癌患者分为生存组(39例)与死亡组(23例)。采用logistic回归分析结肠癌患者预后的影响因素,ROC曲线分析miR-29a、miR-448对结肠癌患者预后死亡的预测效能。结果 结肠癌组织miR-29a、miR-448表达水平均低于癌旁组织(P<0.05);2组肿瘤大小、病理分期、分化程度、淋巴结转移、miR-29a及miR-448表达水平比较,差异有统计学意义(P<0.05);logistic回归显示病理分期、分化程度、淋巴结转移、miR-29a表达水平、miR-448表达水平是结肠癌患者预后死亡的影响因素(P<0.05);ROC曲线显示miR-29a、miR-448及联合预测结肠癌患者死亡的AUC分别为0.705(95%CI:0.575~0.814)、0.710(95%CI:0.580~0.818)、0.732(95%CI:0.605~0.837)。结论 人结肠癌组织中miR-29a、miR-448呈低表达,其水平与结肠癌患者预后有关,miR-29a、miR-448联合检测可提高结肠癌患者死亡的预测效能。Objective This paper aims to investigate the expression of miR-29a and miR-448 in human colon cancer tissues and their relationship with clinical prognosis.Methods Sixty-seven patients with colon cancer admitted to our hospital from January 2019 to December 2019 were selected as the study objects.Clinical data of patients with colon cancer were collected,and the levels of miR-29a and miR-448 were detected from the surgically removed colon cancer tissues and adjacent tissues.The patients with colon cancer were followed up for 6 to 36 months(the 36th month was the end point),excluding 5 cases lost to follow-up,and were divided into survival group(39 cases)and death group(23 cases)according to the prognosis.Logistic regression was used to analyze the prognostic factors of colon cancer patients.ROC curve was used to analyze the predictive efficacy of miR-29a and miR-448 for prognostic death in colon cancer patients.Results The levels of miR-29a and miR-448 in colon cancer tissues were lower than those in adjacent tissues(P<0.05).There was statistical significance on the differences in tumor size,pathological stage,differentiation degree,lymph node metastasis,miR-29a and miR-448 levels between the two groups(P<0.05).Logistic regression showed that pathological stage,degree of differentiation,lymph node metastasis,the level of miR-29a,the expression of miR-448 were the factors affecting prognosis and death of colon cancer patients(P<0.05).ROC curve showed that the AUC of miR-29a,miR-448 and their combination to predict the death of colon cancer patients were 0.705(95%CI:0.575-0.814),0.710(95%CI:0.580-0.818)and 0.732(95%CI:0.605-0.837).Conclusion miR-29a and miR-448 are low expressed in human colon cancer tissues,and their levels are related to the prognosis of patients with colon cancer.Combined detection of miR-29a and miR-448 can improve the predictive efficacy of death in patients with colon cancer.
