Hp感染对胃癌组织中凋亡抑制基因Survivin和MMP-2蛋白表达的影响

摘　　要：目的探讨幽门螺杆菌(Hp)感染对胃癌组织中凋亡抑制基因Survivin和基质金属蛋白酶2(MMP-2)蛋白表达的影响。方法选取2019年6月-2022年6月海口市第四人民医院收治的65例胃癌患者设为胃癌组,另选取139例胃癌前病变患者设为胃癌前病变组(胃黏膜不典型增生66例,胃黏膜肠上皮化生73例),并选取138例慢性胃炎患者作为对照组(慢性萎缩性胃炎71例,慢性浅表性胃炎67例);检测胃癌组织、胃黏膜组织中凋亡抑制基因Survivin、MMP-2蛋白及Hp感染情况,分析Hp感染与胃癌临床特征、凋亡抑制基因Survivin、MMP-2蛋白的关系。结果胃癌组凋亡抑制基因Survivin、MMP-2蛋白阳性表达率>胃癌前病变组>对照组(P<0.05);对照组Hp阳性率低于胃癌组和胃癌前病变组(P<0.05);Hp阳性患者TNM分期为Ⅲ+Ⅳ期、低分化型、淋巴结转移、凋亡抑制基因Survivin阳性、MMP-2蛋白阳性患者占比高于Hp阴性患者;高分化型占比低于Hp阴性患者(P<0.05);Hp感染与胃癌患者TNM分期、淋巴结转移、凋亡抑制基因Survivin阳性表达、MMP-2蛋白阳性表达呈正相关(P<0.05),与分化程度呈负相关(P<0.05)。结论胃癌组织中凋亡抑制基因Survivin和MMP-2蛋白均呈高表达,Hp感染可能通过调控凋亡抑制基因Survivin阳性、MMP-2蛋白表达参与胃癌发生发展。OBJECTIVE To investigate the effect of Helicobacter pylori(Hp)infection on the expression of antiapoptotic gene survivin and matrix metalloproteinase 2(MMP-2)protein in gastric cancer tissues.METHODS A total of 65patients with gastric cancer admitted to Haikou Fourth People′s Hospital from Jun.2019to Jun.2022 were selected as the gastric cancer group,another 139patients with precancerous gastric lesions were selected as the precancerous gastric group(66patients with atypical hyperplasia of gastric mucosa and 73patients with intestinal metaplasia of gastric mucosa),and 138patients with chronic gastritis were selected as the control group(71 patients with chronic atrophic gastritis and 67patients with chronic superficial gastritis).The anti-apoptotic gene surviving,MMP-2protein and Hp infection in gastric cancer tissues and gastric mucosa tissues was detected,and the relationship between Hp infection and clinical features of gastric cancer,anti-apoptotic gene survivin and MMP-2protein was analyzed.RESULTS The positive expression rates of anti-apoptotic gene survivin and MMP-2 protein were the highest in the gastric cancer group,followed by in the precancerous gastric lesion group,and then in the control group(P<0.05).Hp positive rate in the control group was lower than that in the gastric cancer group or the precancerous lesion group(P<0.05).The proportions of patients with TNM stageⅢ+Ⅳ,poor differentiation,lymph node metastasis,anti-apoptotic gene survivin and MMP-2protein in the Hp positive group were higher than those in the Hp negative group.The proportion of highly differentiated types was lower than that in the Hp negative group(P<0.05).Hp infection was positively correlated with TNM stage,lymph node metastasis,positive expression of anti-apoptotic gene survivin and MMP-2protein(P<0.05),and negatively correlated with differentiation degree(P<0.05).CONCLUSION Both the anti-apoptotic gene survivin and MMP-2protein were highly expressed in gastric cancer tissues.Hp infection might be involved in the occurr
