富血小板纤维蛋白联合植骨术对慢性牙周炎骨缺损的效果

摘　　要：目的分析于慢性牙周炎骨缺损中运用富血小板纤维蛋白(platelet rich fibrin,PRF)联合植骨术诊疗的效果及对牙周状况的改善影响。方法选取2021年5月—2022年5月龙岩市第二医院口腔科收治的72例慢性牙周炎骨缺损患者进行研究,采取随机数字表法进行分组。对照组(n=36)以单纯Bio-Oss骨粉完成对骨缺损的修补,治疗组(n=36)采取PRF+Bio-Oss骨粉完成对骨缺损的修补。对比不同修补方法的效果。结果(1)牙周状况:治疗组牙周袋深度、牙槽骨密度等指标优于对照组,差异有统计学意义(P<0.05)。(2)炎症因子水平:治疗组白细胞介素-8(interleukin-8,IL-8)、白细胞介素-6(interleukin-6,IL-6)、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)表达低于对照组,差异有统计学意义(P<0.05)。(3)治疗效果:治疗组总有效率为94.44%,对照组为72.22%,两组对比差异有统计学意义(P<0.05)。(4)临床指标:治疗组探诊深度(probingdepth,PD)、附着丧失(loss ofattachment,AL)等指标优于对照组,差异有统计学意义(P<0.05)。(5)创面愈合状况:随访6个月后,治疗组创面愈合效果优于对照组,差异有统计学意义(P<0.05)。结论于慢性牙周炎骨缺损中运用PRF联合植骨术诊疗效果确切,有益于改善牙周状况,降低炎症反应,促进骨生长,加速创面愈合。Objective To observe and analyze the effect of platelet rich fibrin(PRF)combined with bone grafting on the diagnosis and treatment of bone defects in chronic Periodontal disease and the improvement of periodontal conditions.Methods A total of 72 patients with chronic periodontitis bone defect admitted to the department of stomatology,the Second Hospital of Longyan from May 2021 to May 2022 were studied,and grouped by random number table method.The control group(n=36)completed the repair of bone defects with pure Bio-Oss bone powder,and the treatment group(n=36)completed the repair of bone defects with PRF+Bio-Oss bone powder.The effects of different repair methods were compared.Results(1)Periodontal condition,the treatment group had better periodontal pocket depth,alveolar bone density and other indicators than the control group,with statistically significant differences(P<0.05).(2)Inflammatory factor levels,the expressions of interleukin-8(IL-8),interleukin-6(IL-6),and tumor necrosis factor-α(TNF-α)in the treatment group were lower than those in the control group.The difference was statistically significant(P<0.05).(3)Treatment effect,the total effective rate of the treatment group was 94.44%,while the control group was 72.22%.The difference between the two groups was statistically significant(P<0.05).(4)Clinical indicators,probing depth(PD)and loss of attachment(AL)in treatment group were better than those in control group,the difference was statistically significant(P<0.05).(5)Wound healing status,after a follow-up of 6 months,the treatment group had a better wound healing effect than the control group,with a statistically significant difference(P<0.05).Conclusion The use of PRF plus bone grafting in the treatment of bone defects in chronic periodontal disease is effective,which is beneficial to improve periodontal conditions,reduce inflammatory effects,promote bone growth,and accelerate wound healing.
