上颌前牙区应用根膜技术联合永久冠修复对术后1年种植的临床效果及对修复美学的影响分析

摘　　要：目的:探讨上颌前牙区应用根膜技术联合永久冠修复对术后1年种植的临床效果及修复美学的影响,为临床治疗提供依据。方法:回顾性分析2021年1月—2022年5月黄河三门峡医院收治的125例上颌前牙区牙列缺损患者临床资料,根据治疗方案不同将患者分为研究组(63例,接受根膜技术联合永久冠修复治疗)与对照组(62例,接受永久冠修复治疗),对比两组患者治疗后12个月的红色美学指数(PES)[游离龈水平(FGL)、近中龈乳头(PMG)、远中龈乳头(DGP)、牙龈软组织形态(GSTM)、牙槽骨外形(ABS)、牙龈软组织颜色(GSTC)、牙龈软组织质地(GSTT)]、白色美学指数(WES)[牙冠形态(CM)、牙冠质地(CTe)、牙冠外形(CS)、牙冠颜色(CC)、牙冠透明度(CTr)]及术后并发症。结果:修复后12个月,研究组FGL、PMG、DGP、GSTM、GSTT、GSTC、ABS得分均高于对照组,差异有统计学意义(t=4.335、4.247、3.213、2.631、2.828、3.153、3.201,P<0.05);研究组CM、CTe、CS、CC、CTr得分均高于对照组,差异有统计学意义(t=6.325、5.666、4.713、5.395、5.536,P<0.05);研究组总并发症发生率低于对照组,差异有统计学意义(χ^(2)=4.735,P<0.05)。结论:上颌前牙区应用根膜技术联合永久冠修复的临床效果良好,可有效提高上颌前牙区到缺损患者修复后牙齿美观度并降低牙冠松动、着色、脱落等并发症发生率。Objective:To investigate the effect of applying the root membrane technique combined with permanent crown restoration in the maxillary anterior region on the clinical outcome and restorative aesthetics of implants 1 year after surgery.Methods:The clinical data of 125 patients with tooth loss in the maxillary anterior region admitted to the hospital from January 2021 to May 2022 were retrospectively analyzed,and the patients were divided into a study group(63 patients treated with root membrane technique combined with permanent crown restoration)and a control group(62 patients treated with permanent crown restoration)according to the treatment plan.The pink esthetic score(PES)[Free gingival level(FGL),Papilla of mesial gingiva(PMG),Distal gingival papilla(DGP),Gingival soft tissue morphology(GSTM),Alveolar bone shape(ABS),Gingival soft tissue color(GSTC),Gingival soft tissue texture(GSTT)],white esthetic score(WES)[Crown morphology(CM),Crown texture(CTe),Crown shape(CS),Crown color(CC),Crown transparency(CTr)]and postoperative complications at 12 months after treatment were compared between the two groups.Results:At 12 months after repair,the FGL,PMG,DGP,GSTM,GSTT,GSTC,and ABS scores were higher in the study group than in the control group,with statistically significant differences(t=4.335,4.247,3.213,2.631,2.828,3.153,3.201,P<0.05).The CM,CTe,CS,CC,and CTr scores were higher in the study group than in the control group,with statistically significant differences(t=6.325,5.666,4.713,5.395,5.536,P<0.05).The overall complication rate was lower in the study group than in the control group,with a statistically significant difference(χ^(2)=4.735,P<0.05).Conclusion:The application of the root membrane technique combined with permanent crown restoration in the maxillary anterior region has shown good clinical results,which can effectively improve the aesthetics of the restored teeth and reduce the incidence of complications such as crown loosening,staining and loss.
