摘　　要：缺血性脑卒中(IS)的致残率、致死率较高,已成为全球性疾病。外泌体是近年IS治疗的新靶点,能够作为运输载体穿过血脑屏障,完成细胞间信息交流和遗传物质的转载,参与IS发生发展中的炎症反应、细胞凋亡、血管生成、神经元重塑和突触再生等生物过程,发挥重要通信作用。由外泌体介导的微RNA(miRNA)通过调控基因表达广泛参与生物进程,在IS的诊断、治疗及预后方面表现出巨大潜力。针刺干预IS安全有效,且具有独特优势,可通过调控外泌体miRNA改善脑血流、促进神经发生、抑制细胞凋亡、促进神经元重塑和突触再生,并为进一步探索针刺干预IS的效应机制提供新的分子靶点。Ischemic stroke(IS)has a high disability and mortality rate and has become a global disease.Exosomes are a type of new targets for the treatment of IS in recent years,which can serve as transport carriers to cross the blood-brain barrier,complete intercellular information exchange and genetic material transfer,to participate in biological processes such as inflammation,cell apoptosis,angiogenesis,neuronal remodeling,and synaptic regeneration in the occurrence and development of IS,playing an important communication role.MicroRNA(miRNA)mediated by exosomes are widely involved in biological processes by regulating gene expression,demonstrating great potential in the diagnosis,treatment and prognosis of IS.Acupuncture intervention in IS is safe and effective,and has unique advantages,which can improve cerebral blood flow,promote neurogenesis,inhibit cell apoptosis,promote neuronal remodeling and synaptic regeneration by regulating exocrine miRNA,and provide a new molecular target for further exploring the mechanism of acupuncture intervention in IS.
