摘　　要：十二指肠溃疡是最常见的消化性溃疡疾病,好发于十二指肠球部。出血是十二指肠球部溃疡主要的并发症,慢性出血可降低患者生活质量,短时间内大量急性出血则可能危及患者生命。临床常见的诊断十二指肠球部溃疡出血的方法主要包括胃肠道内镜、实验室及影像学检查等;常见的治疗方法包括内镜、血管介入、外科手术、药物治疗等,内镜下治疗多作为首选疗法,但对于一些整体情况较差的患者需行血管介入或手术治疗,而恰当的药物治疗在各种方案中均具有一定疗效。治疗的最终目标是止血、促进原发病灶愈合以及获得长期稳定疗效。因此,未来的研究方向或将聚焦于止血、持续出血的评估与处理等。Duodenal ulcer is the most common peptic ulcer disease,which is likely to occur in the bulb,and bleeding is the most common complication.Chronic bleeding reduces the quality of life of the patients,and massive acute bleeding in a short time may endanger the patient lives.Gastrointestinal endoscopy,laboratory and imaging examinations are common diagnostic methods for duodenal bulb ulcer bleeding.Common treatment methods,including endoscopy,vascular intervention,surgery and drug therapy,have been widely used in the clinical practice.Endoscopy is often used as the first choice of treatment,but for some patients with poor overall condition,vascular intervention or surgical treatment are needed,while appropriate drug treatment is helpful in various schemes.The ultimate goal of treatment is to stop bleeding,promote the healing of primary lesions,and obtain long-term stable effect.Therefore,future research directions may focus on the evaluation and management of hemostasis and sustained bleeding.
