血清微小RNA-221联合前列腺特异性抗原检测对前列腺癌的诊断及预后评估价值

摘　　要：目的探讨血清微小RNA-221(miR-221)联合前列腺特异性抗原(PSA)检测对前列腺癌的诊断价值,分析血清miR-221与前列腺癌临床病理特征及预后的关系。方法收集2019年1月至2020年6月在宜春市第三人民医院行前列腺穿刺活检术的患者60例,其中病理结果为前列腺癌(前列腺癌组)患者34例、前列腺增生(前列腺增生组)患者26例,测得2组患者的血清miR-221相对表达量及PSA水平,分析miR-221联合PSA检测对前列腺癌的诊断价值,探讨前列腺癌组血清miR-221与患者年龄、前列腺体积、临床分期、分化程度及Gleason评分等临床病理特征的关系。以前列腺癌患者的内分泌治疗去势抵抗时间为指标,分析血清miR-221与前列腺癌患者预后的关系。结果不同Gleason评分、临床分期及分化程度患者血清miR-221相对表达量比较差异有统计学意义(t=7.387,P<0.001;t=7.897,P<0.001;t=7.212,P<0.001)。血清miR-221相对表达量与患者内分泌治疗后的去势抵抗时间呈负相关(r=0.58,P<0.001)。血清miR-221联合PSA检测对前列腺癌的诊断具有一定价值,曲线下面积为0.835(95%CI:0.725~0.945,P<0.001),敏感性为0.78,特异性为0.85。结论血清miR-221联合PSA检测对前列腺癌具有一定的诊断价值,有助于提高单纯PSA检测的特异性;不同临床分期、Gleason评分及分化程度的前列腺癌患者血清miR-221相对表达量存在显著差异;miR-221高表达的前列腺癌患者内分泌治疗去势抵抗时间缩短,预示患者的预后可能较差。Objective To explore the diagnostic value of serum micro RNA-221(miR-221)combined with prostate specific antigen(PSA)detection in prostate cancer,and to analyze the relationship between serum miR-221 and clinicopathological features and prognosis of prostate cancer.Methods Sixty patients underwent prostate biopsy in the Third People’s Hospital of Yichun City,of which 34 patients with prostate cancer(the prostate cancer group)and 26 patients with benign prostatic hyperplasia(the benign prostatic hyperplasia group).Serum miR-221 was measured in the two groups.The expression level and PSA level were analyzed,and the diagnostic value of miR-221 combined with PSA detection in prostate cancer was analyzed,and the relationship between serum miR-221 in the prostate cancer group and the clinicopathological features such as patient age,prostate volume,clinical stage,and Gleason score was explored.At the same time,the relationship between serum miR-221 and the prognosis of patients with endocrine therapy castration resistance time of patients with prostate cancer was used as an indicator.Results There was a statistically significant difference in the serum miR-221 levels among patients with different Gleason score,different clinical stage and different degrees of differentiation((t=7.387,P<0.001;t=7.897,P<0.001;t=7.212,P<0.001));serum miR-221 levels of patients was negatively correlated with the time of castration resistance after endocrine therapy(r=0.58,P<0.001).Serum miR-221 combined with PSA detection had the certain value in the diagnosis of prostate cancer,area under the curve was 0.835(95%CI:0.725-0.945,P<0.001),sensitivity was 0.78,specificity was 0.85.Conclusion Serum miR-221 combined with PSA detection has certain diagnostic value for prostate cancer,which can help to improve the specificity of simple PSA detection;there are significant differences in the serum miR-221 levels of prostate cancer patients with different clinical stages,Gleason scores and degree of differentiation,the castration resistance time of
