摘 要:目的探讨图像增强内镜模式对结肠镜检查中息肉检出效能的应用。方法回顾性分析2019年10月至2021年10月武警北京总队医院收治的484例行结肠镜检查患者的临床资料,按照检查方法分为增强组(n=245)和白光组(n=239),其中增强组再根据不同增强模式细分为联动成像内镜组(n=200)和窄带成内镜组(n=45)。增强组采用白光模式进镜、增强模式退镜;白光组采用白光模式进镜、退镜。比较在不同模式下图像的清晰度、腺瘤检出率(ADR)及息肉特征。结果与白光组比较,增强组图像清晰度更高。各组ADR情况比较,增强组总体ADR为24.49%,白光组总体ADR为21.34%;增强组和白光组比较,在以筛查为目的、≤50岁患者中,增强组ADR检出率高于白光组;增强组内镜撤退时间长于白光组(P<0.05)。NBI组和LCI组比较,LCI组中以筛查为目的、≤50岁患者中ADR检出率高于NBI组(P<0.05)。各组息肉特征比较,增强组息肉检出率为47.34%,病理学诊断为腺瘤84枚;白光组息肉检出率为38.49%,病理学诊断为腺瘤72枚(P<0.05);NBI组和LCI组比较,NBI组炎症性息肉检出率高于LCI组,0~5 cm的腺瘤检出率低于LCI组(P<0.05)。结论图像增强内镜模式(以NBI为主)与白光模式比较,具有较高的清晰度。Objective To investigate the effect of image-enhanced endoscopy on the efficiency of colonoscopic polyp detection.Methods The clinical data of 484 patients admitted to the Beijing General Hospital of Armed Police for colonoscopic examination from October 2019 to October 2021 were retrospectively analyzed,and these patients were divided into an enhanced group(n=245)and a white light group(n=239)according to the examination method,where the enhanced group was further subdivided into narrow-band group(n=200)and linked-color maging group(n=45)according to different enhanced modes.The enhanced group underwent endoscope cannula insertion by the white light mode and removal by the enhanced mode,while the white light group underwent endoscope cannula insertion and removal by the white light mode.The clarity of images,adenoma detection rate(ADR)and polyp characteristics were compared in different modes.Results The image clarity in the enhanced group was higher than that in the white light group.The overall ADR was 24.49% in the enhanced group and 21.34% in the white light group.The ADR in patients≤50 years old for screening purposes in the enhanced group was higher than that in the white light group.The endoscope removal time in the enhanced group was longer than that in the white light group(P<0.05).The ADR in patients≤50 years old for screening purposes in the LCI group was higher than that in the NBI group(P<0.05).The detection rate of polyps in the enhanced group was 47.34%,with 84 adenomas diagnosed by pathology,while the detection rate of polyps in the white light group was 38.49%,with 72 adenomas diagnosed by pathology(P<0.05).The detection rate of inflammatory polyps was higher,while the detection rate of 0-5 cm adenomas was lower in the NBI group than in the LCI group(P<0.05).Conclusion The image clarity under the image-enhanced endoscopy(with NBI as the main mode)is higher than that under white light endoscopy.
