足拇趾甲下鳞状细胞癌1例

摘　　要：患者男,73岁,右足拇趾趾甲病变10年,近1年甲床反复感染,脓性分泌物,创面不能愈合。皮损组织病理示:皮肤溃疡性病变,可见肉芽组织及鳞状上皮增生,部分鳞状上皮细胞巢团边缘不规则、基底膜不清,于间质内呈浸润性生长,巢团内细胞极向紊乱,细胞异型。免疫组化结果:p53弥漫强阳性,呈突变型表达,S100(-),HMB-45(-),诊断:鳞状细胞癌。A 73-year-old male patient had nail lesions on the right toe for 10 years.In the past 1 year,the nail bed was repeatedly infected,purulent secretions,and the wound could not heal.Skin histopathology showed skin ulcerative lesions,granulation tissue and squamous epithelial hyperplasia.Some squamous epithelial cell nests had irregular edges,unclear basement membrane,and invasive growth in the stroma.The cells in the nests were extremely disordered and the cells were atypia.Immunohistochemistry examination showed:p53 diffuse strong positive,showing mutant expression,S100(-),HMB-45(-),diagnosis:squamous cell carcinoma.
