摘　　要：目的探讨非离断式Roux-en-Y吻合术在胃癌消化道重建中的应用效果及对胃肠道屏障功能、胆囊收缩功能的影响。方法选取2020年2月至2022年2月收治的40例行胃癌消化道重建手术患者,按照吻合方法不同将其分为对照组和观察组,各20例。对照组采用传统Roux-en-Y吻合术,观察组采用非离断式Roux-en-Y吻合术。比较两组的治疗效果。结果术后1 d,观察组的血浆D-乳酸、二胺氧化酶(DAO)水平均低于对照组(P<0.05)。术后,观察组的双歧杆菌、乳杆菌、拟杆菌数量多于对照组(P<0.05)。术后1 d,观察组的胆囊收缩素水平低于对照组,胆囊容积、胆囊排空率高于对照组(P<0.05)。观察组的并发症总发生率低于对照组(P<0.05)。结论非离断式Roux-en-Y吻合术应用于胃癌消化道重建中有助于改善患者的胃肠道屏障功能、胃肠道微生态平衡及胆囊收缩功能,减少并发症发生。Objective To investigate the application effect of nondisjunction uncut Roux-en-Y anastomosis in digestive tract reconstruction of gastric cancer and its influences on gastrointestinal barrier function and gallbladder contraction function.Methods A total of 40 patients underwent digestive tract reconstruction for gastric cancer from February 2020 to February 2022 were selected and divided into control group and observation group according to different anastomosis methods,with 20 cases in each group.The control group was treated with traditional Roux-en-Y anastomosis,and the observation group was treated with nondisjunction uncut Roux-en-Y anastomosis.The therapeutic effects of the two groups were compared.Results At 1 d after operation,the levels of plasma D-lactic acid and diamine oxidase(DAO)in the observation group were lower than those in the control group(P<0.05).After operation,the number of Bifidobacterium,Lactobacillus and Bacteroides in the observation group were more than those in the control group(P<0.05).At 1 d after operation,the level of cholecystokinin in the observation group was lower than that in the control group,and the gallbladder volume and gallbladder emptying rate were higher than those in the control group(P<0.05).The total incidence of complications in the observation group was lower than that in the control group(P<0.05).Conclusion The nondisjunction uncut Roux-en-Y anastomosis in the digestive tract reconstruction of gastric cancer is helpful to improve the gastrointestinal barrier function,gastrointestinal microecological balance and gallbladder contraction function of patients,and reduce the occurrence of complications.
