摘　　要：目的:探讨经腹部联合经阴道彩色多普勒超声在帆状胎盘合并血管前置产前诊断中应用价值。方法:回顾性分析2020年1月至2022年12月该院收治的可能存在胎盘或脐带异常的93例孕妇的临床资料,所有孕妇均行经腹部与经阴道彩超检查,以产后胎盘检查结果为金标准,分析经腹部、经阴道彩超单独及联合检查对帆状胎盘、帆状胎盘合并血管前置的诊断价值。结果:产后胎盘诊断证实,93例产妇中检出帆状胎盘56例,检出率为60.22%,其中帆状胎盘合并血管前置39例,检出率为41.94%。经腹部联合经阴道彩超诊断帆状胎盘的灵敏度、特异度、准确度分别为92.86%、94.59%、93.55%,诊断帆状胎盘合并血管前置的灵敏度、特异度、准确度分别为94.87%、94.44%、94.62%,均高于经腹部、经阴道彩超单独检查,差异有统计学意义(P<0.05)。结论:经腹部联合经阴道彩超诊断帆状胎盘合并血管前置的价值高于二者单独检查,对提高该病检出率、及时干预及改善妊娠结局意义重大。Objective:To explore application value of transabdominal combined with transvaginal color Doppler ultrasound examination in diagnosis of velamentous placenta combined with vasa previa.Methods:The clinical data of 93 pregnant women with possible placental or umbilical cord abnormalities admitted to the hospital from January 2020 to December 2022 were retrospectively analyzed.All pregnant women underwent transabdominal and transvaginal color Doppler ultrasound examinations.Using the results of postpartum placental examination as the gold standard,the diagnostic value of single transabdominal color Doppler ultrasound examination,single transvaginal color Doppler ultrasound examination and combined examination for velamentous placenta and velamentous placenta combined with vasa previa was analyzed.Results:The postpartum placental diagnosis confirmed that 56 cases of velamentous placenta were detected in the 93 pregnant women with a detection rate of 60.22%.Among them,39 cases of velamentous placenta combined with vasa previa were detected with a detection rate of 41.94%.The sensitivity,the specificity and the accuracy of transabdominal combined with transvaginal color Doppler ultrasound examination in the diagnosis of velamentous placenta were 92.86%,94.59%and 93.55%,separately.The sensitivity,the specificity and the accuracy of transabdominal combined with transvaginal color Doppler ultrasound examination for velamentous placenta combined with vasa previa were 94.87%,94.44%and 94.62%,separately,which were higher than those of single transabdominal color Doppler ultrasound examination and single transvaginal color Doppler ultrasound examination,and the differences were statistically significant(P<0.05).Conclusions:The value of transabdominal combined with transvaginal color Doppler ultrasound examination in the diagnosis of velamentous placenta combined with vasa previa is higher than that of two single examinations,which is of great significance to improve the detection rate of the disease,timely give the interventi
