角膜溃疡清创术联合抗真菌药物治疗真菌性角膜炎的效果观察

摘　　要：目的:分析角膜溃疡清创术联合抗真菌药物治疗真菌性角膜炎的效果。方法:选取2019年1月—2023年5月黔西南州人民医院收治的200例真菌性角膜炎患者作为研究对象,根据随机数字表法分为对照组与观察组,各100例。两组均行常规治疗,对照组治疗在常规治疗基础上给予抗真菌药物治疗,观察组在对照组基础上实施角膜溃疡清创术。比较两组治疗效果。结果:观察组治疗总有效率高于对照组,差异有统计学意义(P=0.003)。观察组畏光流泪、眼部异物、角膜溃疡改善时间短于对照组,差异有统计学意义(P<0.001)。两组不良反应总发生率比较,差异无统计学意义(P>0.05)。治疗后,两组生理功能、心理功能、精神健康、躯体疼痛评分高于治疗前,且观察组高于对照组,差异有统计学意义(P<0.001)。结论:角膜溃疡清创术联合抗真菌药物治疗真菌性角膜炎的效果显著,可促进患者病情改善,提高患者生活质量,且不会增加不良反应。Objective:To analyze the effect of corneal ulcer debridement and antifungal drug in treatment of fungal keratitis.Methods:A total of 200 patients with fungal keratitis admitted to Qianxinan Prefecture People's Hospital from January 2019 to May 2023 were selected as the study subjects.They were divided into a control group and an observation group according to random number table method,with 100 cases in each group.Both groups received conventional therapy.The control group received antifungal drug treatment on the basis of conventional therapy,while the observation group received corneal ulcer debridement on the basis of control group.The treatment effect was compared between two groups.Results:The total effective rate of treatment in the observation group was higher than that in the control group,and the difference was statistically significant(P=0.003).The photophobia,tearing,ocular foreign body and corneal ulcer improvement time in the observation group was shorter than that in the control group,and the difference was statistically significant(P<0.001).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).After treatment,the scores of physiological function,psychological function,mental health and physical pain in the two groups were higher than those before treatment,and the scores in the observation group were higher than those in the control group,and the difference was statistically significant(P<0.001).Conclusion:Corneal ulcer debridement combined with antifungal drugs in the treatment of fungal keratitis has a significant effect,which can promote the improvement of patients'disease condition and improve their quality of life,and will not increase adverse reactions.
