摘  要:目的:探究大鼠椎间融合模型的建立以及力学测试。方法:2019年1月~2021年5月期间选取纯种健康雄性SD大鼠60只,随机分为三组,各20只,制备椎间融合模型,取双侧髂骨制成颗粒骨,A组:植入自体骨量0.3 cm^(3),B组:植入自体骨量0.2 cm^(3),C组:不植骨(空白对照组)。植入后分别予以放射学检查、生物力学检测及组织学观察,分析不同融合骨量对骨融合率、强度影响。结果:4 w手触检查显示:A组融合率为1只(10.00%),B组、C组未出现融合,6 w后A组融合4只(40.00%),B组融合为4只(40.00%),C组为0只(0.00%)。组织学中A组与B组融合组织中,表现为丰富骨桥,C组以大量排列紊乱胶原纤维为主,未见到新生骨组织。两组鼠骨钙素(BGP)、骨碱性磷酸酶(BALP)、转化生长因子β(TGF-β)、骨形态蛋白(BMP-2)、血管内皮生长因子VEGF)水平分别在术后0、3、5、7、14、30 d比较差异无统计学意义(P>0.05),A组、B组术后2 w、术后3 w、术后4 w X线评分高于C组,差异有统计学意义(P<0.05)。结论:本次试验模型应用手术方式和融合技术,并发症少及成功率高,与临床椎间融合相符。Objective To establish the lumbar interbody fusion models of rats and mechanical testing.Method From January 2019 to May 2021,60 purebred healthy male SD rats were selected and randomly divided into 3 groups,with 20 rats in each group.Lumbar interbody fusion model was prepared,bilateral ilium was made into particles.Group A was implanted with 0.3 cm^(3) of autogenous bone,group B with 0.2 cm^(3) of autogenous bone and group C without bone graft(Blank control group).After implants,radiological examination,biomechanical detection and histological observation were performed.Impact of fusion bone mass on bone fusion rate and strength was evaluated.Results After 4w of examination,group A had one fusion case(10.00%),but without fusion cases in group B and group C;after 6w,fusion rate in group A was 40.00%(4 cases),40.00%(4 cases)in group B and 0.00%in group C(0 case).Histological examination indicated that group A and group B had a plenty of bone bridges and group C with lots of unorderly collagenous fiber and without new bone tissues.After 0d,3d,5d,7d,14d and 30d of surgery,bone gla-protein(BGP),bone alkaline phosphatase(BALP),transforming growth factor-β(TGF-β),bone Morphogenetic protein-2(BMP-2)and vascular endothelial growth factor(VEGF)among groups were not significantly different(P>0.05);after 2w,3w and 4w of surgery,X-ray image diagnostic scores in group A and group B were significantly higher than group C(P<0.05).Conclusion The surgical treatment and fusion techniques can reduce the complications and increase the success rate of interbody fusion.
