摘 要:目的:基于冰山模型构建互联网诊疗医生胜任力评价指标体系并分析相关影响因素,为提升从事互联网诊疗的医生胜任力提供参考。方法:采用文献分析法和改良德尔菲法,于2022年2-4月对来自卫生行政部门、医院以及科研中心等机构的21名专家进行两轮咨询,对互联网诊疗医生胜任力评价指标进行筛选,使用层次分析法计算指标权重。结果:构建了包含4个一级指标、8个二级指标、35个三级指标的互联网诊疗医生胜任力评价指标体系。其中一级指标及权重分别为:知识技能(0.46)、沟通协作(0.31)、个人特质(0.09)和辅助能力(0.14)。二级维度包含:专业知识(0.61)、临床技能(0.27)、软件操作能力(0.12)、线上沟通协作能力(0.67)、线上线下协作诊疗能力(0.33)、线上诊疗需要的个人特质(1.00)、学习优化能力(0.20)及相关政策法律了解能力(0.80)。结论:评价互联网诊疗医生胜任力既要重视“知识技能”等显性要素,也要重视“沟通协作”等隐性要素,未来应对从事互联网诊疗的医生进行多维、全面的培养与胜任力考核。Objective:Based on the iceberg model,this study aims to construct a competency evaluation indicator system for doctors engaged in internet medical practice and to analyze the relevant influencing factors.This research seeks to provide a reference for enhancing the competencies of doctors involved in online medical consultations.Methods:Literature analysis and a modified Delphi method were employed.From February to April 2022,two rounds of consultations were conducted with 21 experts from health administration departments,hospitals,and research centers.These consultations aimed to filter the competency evaluation indicators for doctors engaged in internet medical practice.The Analytic Hierarchy Process(AHP)was used to calculate the weights of the indicators.Results:A competency evaluation indicator system for internet medical practitioners was developed,comprising 4 primary indicators,8 secondary indicators,and 35 tertiary indicators.The primary indicators'weights were as follows:knowledge and skills(0.46),communication and collaboration(0.31),personal traits(0.09),and supplementary abilities(0.14).Secondary dimensions included:professional knowledge(0.61),clinical skills(0.27),software operating capability(0.12),online communication and collaboration skills(0.67),combined online-offline clinical collaboration(0.33),personal traits needed for online consultation(1.00),learning and optimization abilities(0.20),and understanding of relevant policies and laws(0.80).Conclusion:When evaluating the competencies of doctors engaged in internet medical practice,both overt elements like"Knowledge and Skills"and covert aspects such as"Communication and Collaboration"should be emphasized.Future endeavors should focus on providing multi-dimensional,comprehensive training and competency assessments for these practitioners.
