基于“五志过极”理论探讨功能性胃肠病的病因病机

摘　　要：功能性胃肠病(FGIDs)是一组临床常见的胃肠道疾病,其发病率、患病率逐年攀升,由于该类疾病症状重叠、反复发作、病程迁延,严重影响了患者的生活质量。FGIDs的发病机制尚未完全阐明,当前研究表明精神心理因素在FGIDs的发病中占据重要地位。西医尚无针对FGIDs的特效药,中医从情志致病角度辨治本病临证效果颇佳,而“五志过极”理论是中医情志致病的重要组成部分。本文从“五志过极”理论出发,追溯了“五志过极”的理论源流及古今医家的临证应用,认为“五志过极”是诱发FGIDs的重要病因,“五志过极”导致脏腑功能失常则是FGIDs的基本病机,分析了“五志过极”和现代医学“脑肠互动紊乱”之间的同一性,以期为中医药辨治FGIDs,改善患者临床症状提供新的诊疗思路及方向。Functional gastrointestinal disease(FGIDS)is a set of clinical gastrointestinal diseases.Its incidence and prevalence have risen year by year.Due to the overlapping symptoms of such diseases,recurrence,and delay,it has seriously affected the life of patients lives quality.The pathogenesis of FGIDS has not been fully stated but the current research shows that psychological and psychological factors occupy an important position in the onset of FGIDS.Western medicine has no special effects on FGIDS.Traditional Chinese Medicine has a good effect from the perspective of emotional pathogenesis.This article starts from the theory of"five excessive emotions",tracing the theoretical source flow of"five excessive emotions"and the application of ancient and modern medical doctors.It is the basic pathogenesis of FGIDS,and analyzes the same nature between the"five excessive emotions"and the modern medicine"Brain Interaction Disorders",to provide new gastrointestinal diseases in Traditional Chinese Medicine and improve the clinical symptoms of patients.It is expected to provide new diagnoses treatment ideas and directions for patients with the functional gastrointestinal disease of Chinese medicine and improve the clinical symptoms of patients.
