摘  要:目的应用光学相干断层扫描血管成像技术(optical coherence tomography angiography,OCTA)检测不同程度近视人群的黄斑区视网膜血管密度(vascular density,VD)及厚度(retinal thickness,RT),并对其进行相关性分析。方法纳入18~35岁近视患者共200例,其中145眼有效,根据等效球镜度(spherical equivalent refraction,SER)将其分为高度近视组(SER<-6.00 D)57眼、中度近视组(-6.00~-3.00)43眼、轻度近视组(-3.00~-0.50)45眼。对所有受检者进行OCTA检查,量化黄斑区各部位视网膜厚度及表层血管密度,比较轻、中及高度近视组各部位视网膜微血管密度及视网膜厚度的差异,并进行相关性分析。结果轻度近视、中度近视的浅表和深部黄斑血管密度无统计学差异,中度近视和高度近视组浅层和深部黄斑血管密度均与平均动脉压相关。此外,黄斑区视网膜厚度与浅层黄斑血管密度满足正相关。结论不同程度的近视不影响健康成人的黄斑血管密度与黄斑区视网膜厚度。此外,黄斑浅层血管密度作为独立因素与黄斑神经节细胞复合体厚度呈正相关。Objective To detect the vascular density(VD)and retinal thickness(RT)in macular area of people with different degrees of myopia with optical coherence tomography angiography(OCTA),and to analyze the correlation.Methods 200 patients with myopia aged 18~35 years old were enrolled.Among them,145 eyes were effective.According to spherical equivalent refraction(SER),they were divided into the high myopia group(SER<-6.00 D)57 eyes,the moderate myopia group(-6.00~-3.00)43 eyes,and the mild myopia group(-3.00~-0.50)45 eyes.OCTA examination was performed on all subjects to quantify the retinal thickness and surface microvascular density in each part of the macular area.The differences of retinal microvascular density and retinal thickness in each part of the control group and the high myopia group were compared and the correlation was analyzed.Results There were no significant differences in the superficial and deep macular vascular density between mild myopia and moderate myopia.The superficial and deep macular vascular density in the moderate and high myopia groups was correlated with mean arterial pressure.In addition,the macular retinal thickness was positively correlated with the superficial macular vascular density.Conclusion Different degrees of myopia do not affect the macular vascular density and macular retinal density in healthy adults.In addition,superficial macular vascular density as an independent factor was positively correlated with the thickness of macular ganglion cell complex.
