摘 要:目的:探讨初治弥漫大B细胞淋巴瘤(DLBCL)患者组织中核酸外切体成分4(EXOSC4)的表达情况及其临床意义。方法:采用免疫组织化学染色方法检测181例初治DLBCL患者组织中EXOSC4蛋白的表达情况;收集临床资料,并分析初治DLBCL患者组织中EXOSC4蛋白表达与临床特征的相关性及其预后意义。结果:181例初治DLBCL患者组织中,EXOSC4蛋白表达阳性率为68.51%;将患者分为高表达组44例和低表达组137例,两组患者年龄、性别、B症状、血清乳酸脱氢酶(LDH)水平、美国东部肿瘤协作组(ECOG)评分、Ann Arbor分期、结外侵犯、国际预后指数(IPI)评分、国家综合癌症网络IPI(NCCN-IPI)评分、细胞起源等特征的差异均无统计学意义(P>0.05)。Cox多因素回归分析结果显示,组织EXOSC4蛋白高表达是初治DLBCL患者OS和PFS的独立不良预后因素(均P<0.05)。K-M生存分析结果显示,组织中EXOSC4蛋白高表达初治DLBCL患者总生存期和无进展生存期较低表达患者显著缩短(均P<0.05)。结论:初治DLBCL患者组织EXOSC4蛋白高表达是影响生存的独立不良预后因素。Objective:To explore the expression of Exosome Component4(EXOSC4)in the tissues of newly diagnosed patients with diffuse large B-cell lymphoma(DLBCL)and its clinical significance.Methods:The expression of EXOSC4protein in the tissues of181newly diagnosed DLBCL patients was analyzed by immunohistochemical staining.Clinical data were collected.The correlation between EXOSC4protein expression in the tissues of newly diagnosed DLBCL patients and clinical features were analyzed and its prognostic significance.Results:The positive rate of EXOSC4protein expression was68.51% in the tissues of181newly diagnosed DLBCL patients.These patients were divided into two groups,with44cases in high expression group and137cases in low expression group.There were no significant differences in age,gender,B symptoms,serum lactate dehydrogenase(LDH)level,Eastern Cooperative Oncology Group(ECOG)score,Ann Arbor stage,extranodal disease,International Prognostic Index(IPI)score,National Comprehensive Cancer Network IPI(NCCN-IPI)score,and cell origin between the two groups(P<0.05).Cox multivariate regression analysis showed that high EXOSC4protein expression in tissues was an independent poor prognostic factor for OS and PFS in newly diagnosed DLBCL patients(all P<0.05).K-M survival analysis showed that newly diagnosed DLBCL patients with high EXOSC4protein expression had significantly shorter overall survival(OS)and progression free survival(PFS)than those patients with low EXOSC4protein expression(all P<0.05).Conclusion:High EXOSC4protein expression in tissues of newly diagnosed DLBCL patients is an independent poor prognostic factor for survival.
