肾上腺皮质腺瘤伴下腔静脉瘤栓1例报告

摘　　要：回顾1例肾上腺皮质腺瘤合并下腔静脉瘤栓患者的临床资料。患者女,64岁,因间断头晕4个月入院。术前CT检查提示右肾上腺肿瘤。拟行后腹腔镜右肾上腺切除术,术中发现合并下腔静脉瘤栓,改行右肾上腺切除术+瘤栓取出术。术后病理诊断为肾上腺皮质腺瘤合并下腔静脉瘤栓。肾上腺皮质腺瘤需与皮质癌相鉴别,术前行下腔静脉彩色多普勒超声检查、CT血管造影或下腔静脉造影可明确是否伴下腔静脉瘤栓并行瘤栓分级,根据瘤栓分级可选择不同的手术方式。The clinical data of a 64-year-old patient with adrenocortical adenoma complicated with inferior vena cava tumor thrombus(IVCTT)were retrospectively analyzed.The patient was admitted becourse of intermittent dizziness for 4 months.CT examination revealed right adrenal tumor,and IVCTT was found in operation.Adrenal cortical adenoma needs to be distinguished from adrenal cortical carcinoma pathologically.Preoperative color Doppler ultrasonography,CT angiography or inferior vena cava angiography can confirm the diagnosis of IVCTT and tumor thrombus grade,and different surgical methods should be selected according to tumor thrombus grade.
