摘　　要：目的探讨恶性肿瘤病人碘对比剂急性不良反应的相关因素及临床特点分析。方法回顾性分析2018年5月至2021年10月北京市朝阳区三环肿瘤医院行增强CT的50100例恶性肿瘤病人,其中33例出现碘对比剂急性不良反应,分析其临床特点和相关因素。结果增强CT检查中碘对比剂急性不良反应的总体发生率为0.065%(33/50100),重度不良反应发生率占0.01%(6/50100)。女性发生率为0.0961%(20/20804)显著高于男性发生率0.0444%(13/29263)(P<0.05)。不同年龄组比较,≤35岁年轻病人易发生急性不良反应(P<0.05)。33例中既往有食物及药物过敏史6例,高血压病史5例。急性不良反应发生时间范围1~25 min;不良反应症状主要表现为荨麻疹、恶心呕吐、高血压、呼吸困难及过敏性休克等;其中皮肤系统不良反应最高(60.6%),其次消化、循环、呼吸系统。给予抗过敏治疗均得到有效控制。结论恶性肿瘤病人在增强CT检查时,可能会出现不同程度的碘对比剂急性不良反应,女性、年轻病人发生率较高,医护人员对肿瘤病人检查前进行准确评估,严格筛选,提高警惕,确保临床用药安全。Objective To investigate the related factors and clinical characteristics of acute adverse reactions to iodine contrast in patients with malignant tumors.Methods This was a retrospective analysis of 50100 patients with malignant tumors who underwent enhanced CT at Beijing Sanhuan Cancer Hospital from May 2018 to October 2021,in which 33 patients developed acute adverse reactions to iodine contrast,and the clinical characteristics and related factors of the patients with adverse reactions were analyzed.Results The overall incidence of acute adverse reactions of enhanced CT was 0.065%(33/50100),and the incidence of severe adverse reactions was 0.01%(6/50100).The incidence was 0.0961%(20/20804)in females,and 0.0444%(13/29263)in males,which was significantly higher(P<0.05).Comparing different age groups,young patients under 35 years old were prone to acute adverse reactions(P<0.05).Among the 33 patients,there was a history of previous food and drug allergy in 6 cases,and a history of hypertension in 5 cases.The time range of acute adverse reactions to iodine contrast agents was 1-25 min.The symptoms of adverse reactions mainly manifested as urticaria,nausea and vomiting,hypertension,dyspnea and anaphylactic shock.The adverse reaction of the skin system was the highest(60.6%),followed by the digestive,circulatory and respiratory systems.Antiallergic treatment was effectively controlled.Conclusions Malignant tumor patients may have adverse reactions to varying degrees when enhancing CT,and the incidence is higher in female and young patients.Medical staff should accurately evaluate tumor patients before examination,strictly screen them,and be more vigilant to ensure the safety of clinical medication.
