摘 要:目的 探讨血液不同分离结果的多发性骨髓瘤(MM)患者IL-17、IL-35、β2-MG的表达水平及临床意义.方法 分析2016年10月-2022年3月南昌大学第二附属医院收治的143例初诊MM患者的临床资料,根据分离胶采血管血液离心后的结果,将MM患者分为对照组(自上而下依次为血清、分离胶、红细胞)和异常组(自上而下依次为血清、红细胞与血清的混合层、胶体、红细胞),比较治疗前后两组患者血液分离结果的变化情况及IL-17、IL-35、β2-MG水平的差异;并分析β2-MG与IL-17、IL-35水平的相关性.结果 143例MM患者的血液标本离心后有22例出现血液异常分离现象,异常分离率为15.38%.异常组患者IL-17、β2-MG水平显著高于对照组,IL-35水平显著低于对照组(P<0.05).对照组121例患者治疗后有115例血液分离结果未发生变化,有6例患者血液分离结果转为异常;其中血液分离结果未发生变化的患者IL-17、β2-MG水平明显降低,IL-35水平显著升高(P<0.05);转为异常的患者IL-17、β2-MG水平明显升高,IL-35水平显著降低(P<0.05).异常组22例患者治疗后有16例血液分离结果转为正常,有6例患者血液分离结果没有发生变化;其中血液分离结果转为正常的患者IL-17、β2-MG水平明显降低,IL-35水平显著升高(P<0.05),血液分离结果未发生变化的患者IL-17、IL-35、β2-MG水平与治疗前相比无明显变化(P>0.05).经Pearson相关分析显示,MM患者血清β2-MG水平与IL-17水平呈正相关(r=0.702,P<0.001),与IL-35水平呈负相关(r=-0.713,P<0.001).结论 不同血液分离结果的MM患者IL-17、IL-35、β2-MG等指标水平存在差异,血液分离结果的转变联合IL-17、IL-35、β2-MG水平的变化可作为评估MM患者病情及疗效的重要指标.Objective To investigate the expression the levels of IL-17,IL-35,β2-MG and clinical significance.Methods The clinical data of 143 newly diagnosed patients with MM admitted to our hospital from October 2016 to March 2022 were analyzed.According to the results of blood centrifugation of gel separated blood collection vessels,MM patients were divided into control group(from top to bottom:serum,gel separated,red blood cells)and abnormal group(from top to bottom:serum,mixed layer of red blood cells and serum,colloid,red blood cells).The changes of blood separation results and differences in IL-17,IL-35,β2-MG levels;and analyze the correlation betweenβ2-MG and IL-17,IL-35 levels.Results After centrifugation,22 out of 143 blood samples from MM patients showed abnormal blood separation,with a rate of 15.38%.Abnormal group patients the IL-17,β2-MG level was significantly higher than the control group,while the IL-35 level was significantly lower than the control group(P<0.05).After treatment,115 out of 121 patients in the control group showed no changes in blood separation results,while 6 patients showed abnormal blood separation results;among them,patients with blood separation results unchanged the level of IL-17,β2-MG significantly decreased,while the level of IL35 significantly increased(P<0.05);transforming into abnormal patient the level of IL-17,β2-MG significantly increased,while the level of IL-35 significantly decreased(P<0.05).After treatment,16 out of 22 patients in the abnormal group showed normal blood separation results,while 6 patients showed no changes in blood separation results;among them,patients with normal blood separation results showed IL-17,β2-MG levels significantly decreased,IL-35 levels significantly increased(P<0.05),there was no significant change in IL-17,IL-35 andβ2-MG levels compared to before treatment(P>0.05).According to Pearson correlation analysis,the serum of MM patientsβ2-MG levels were positively correlated with IL-17 levels(r=0.702,P<0.001),and negatively correlated with
