布地奈德福莫特罗粉吸入剂致喘憋加重1例

摘　　要：1例71岁女性患者,既往有牛奶、牛肉过敏史。2021年8月诊断为支气管哮喘。2021年12月起予布地奈德福莫特罗粉吸入剂(2吸,bid)治疗,使用2 d后患者喘憋症状加重,无法平卧,行平路感喘憋,停用后症状好转。2022年2月因“咳嗽、咳痰、喘憋18余月”入院,药师考虑患者吸入布地奈德福莫特罗粉吸入剂后喘憋加重的原因可能为辅料乳糖(含少量牛乳蛋白)引起的过敏反应,建议更换为倍氯米松福莫特罗吸入气雾剂,但患者担心喘憋加重未使用。2022年4月该患者就诊于外院,再次使用布地奈德福莫特罗粉吸入剂后喘憋加重,停用后好转。2022年5月再次入我院,予以倍氯米松福莫特罗吸入气雾剂治疗,患者哮喘症状得到良好的控制。A 71-year-old female patient with a history of allergies to milk and beef was diagnosed with bronchial asthma in August 2021.In December 2021,she was given budesonide and formoterol fumarate powder for inhalation(2 inhalers each time,2 times a day).After 2 days,she developed worsened dyspnea,inability to lie flat,dyspnea after walking on a flat road,which improved after stopping budesonide and formoterol fumarate powder for inhalation.In February 2022,she was admitted to our hospital due to cough,phlegm and dyspnea for more than 18 months.Pharmacists considered that the excipient lactose(containing a small amount of cow milk protein)in budesonide and formoterol inhalation powder for inhalation might be the reason for the allergic reaction,and suggested giving beclometasone dipropionateand formoterol inhalation aerosol.However,she was worried about asthma aggravation and refused.In April 2022,she was treated with budesonide and formoterol fumarate powder for inhalation and developed asthma aggravation again in another hospital,which also improved after discontinuation.In May 2022,she was admitted to our hospital again and treated with beclometasone dipropionateand formoterol inhalation aerosol,which resulted in well-controlled asthma.
