术前胃镜活检诊断胃癌的效果分析

摘　　要：目的分析术前胃镜活检诊断胃癌的效果。方法104例接受手术治疗的胃癌患者,均接受术前胃镜活检和术后病理检查,以术后病理检查为金标准,分析术前胃镜活检的诊断结果及对胃癌类型的检出情况。结果术前胃镜活检的诊断准确率为93.27%(97/104)。术前胃镜活检的分化良好检出率29.81%(31/140)低于术后病理检查的45.19%(47/104),差异有统计学意义(P<0.05)。术前胃镜活检对黏液腺癌、乳头状腺癌、管状腺癌、印戒细胞癌检出符合率分别为9.09%(1/11)、85.37%(35/41)、32.26%(10/31)、85.71%(18/21)。结论术前胃镜活检能够为胃癌的鉴别和手术方案制定提供可靠参考,但是进行全面评价还需要参照术后病理结果完成,值得重视。Objective To analyze the effect of preoperative gastroscopic biopsy in the diagnosis of gastric cancer.Methods All 104 cases of gastric cancer patients underwent preoperative gastroscopic biopsy and postoperative pathological examination.Pathological findings were used as the gold standard to analyze the results of preoperative gastroscopic biopsy and the detection of gastric cancer types.Results The diagnostic accuracy rate of preoperative gastroscopic biopsy was 93.27%(97/104).The detection rate of well-differentiation by preoperative gastroscopic biopsy was 29.81%(31/104),which was lower than that of 45.19%(47/104)of postoperative pathological examination,and the difference was statistically significant(P<0.05).The compliance rates of preoperative gastroscopic biopsy for mucinous adenocarcinoma,papillary adenocarcinoma,tubular adenocarcinoma,and signet-ring cell carcinoma were 9.09%(1/11),85.37%(35/41),32.26%(10/31),and 85.71%(18/21),respectively.Conclusion Preoperative gastroscopic biopsy can provide a reliable reference for identification and surgical plan formulation of gastric cancer,but the comprehensive evaluation needs to be completed with reference to postoperative pathological findings,which deserves attention.
