摘 要:目的:探讨肺癌患者维生素D的表达、免疫功能关联性,研究通过调整患者体内维生素D水平来调节患者自身免疫功能,以此提高肺癌患者临床治疗效果的新策略的可行性和有效性。方法:本研究选取2022年1月至12月期间到佛山市高明区人民医院就诊的60名肺癌确诊患者为研究对象,作为研究组,另选取60名健康人员作为对照组。使用全自动化学发光分析技术检测所有参与者血清中的维生素D水平,使用免疫芯片技术测定外周血免疫细胞计数并计算比值。通过对患者进行维生素D补充治疗,探讨维生素D的表达对免疫细胞的调节作用。结果:肺癌患者维生素D的水平与健康人员比较有统计学差异(P<0.05)[研究组vs对照组:(23.1±8.5)μg/L vs(74.5±21.8)μg/L]。以维生素D为因变量行多元线性回归分析显示:患者的维生素D水平与血红蛋白(HB)、白蛋白(ALB)、前白蛋白(PA)水平呈正相关,与尿蛋白水平呈负相关。蛋白芯片分析结果显示:研究组CD4、CD8水平为(0.425±0.021)%、(0.451±0.032)%,CD4/CD8为(0.92±1.02);对照组CD4、CD8水平为(0.465±0.141)%、(0.347±0.036)%,CD4/CD8为(1.32±0.22);两组对应指标比较,差异有统计学意义(均P<0.05)。结论:肺癌患者维生素D的表达显著下调,为其补充调整体内维生素D的水平可能有助于调节其免疫细胞的表达,这为低水平维生素D肺癌患者的治疗提供了新思路(改善免疫细胞水平,进而提高其临床治疗效果)。Objective:To explore the correlation between the expression of vitamin D and immune function in patients with lung cancer,and to study the feasibility and effectiveness of a new strategy to regulate the autoimmune function of patients by adjusting the level of vitamin D in patients with lung cancer,so as to improve the clinical treatment effect of patients with lung cancer.Methods:In this study,60 patients diagnosed with lung cancer who visited Gaoming District People’s Hospital of Foshan City from January to December 2022 were selected as the study group,and 60 healthy people were selected as the control group.Serum vitamin D levels in all participants were measured using automated chemiluminescence analysis,and peripheral blood immune cell counts were measured using immunochip technology and ratios were calculated.To explore the regulatory effect of vitamin D expression on immune cells by supplementing patients with vitamin D.Results:The level of vitamin D in lung cancer patients was significantly different from that in healthy people(P<0.05)(study group vs control group:(23.1±8.5)μg/L vs(74.5±21.8)μg/L).Multiple linear regression analysis with vitamin D as the dependent variable showed that vitamin D levels were positively correlated with hemoglobin(HB),albumin(ALB)and prealbumin(PA)levels,and negatively correlated with urinary protein levels.Protein microarray analysis showed that CD4 and CD8 levels were(0.425±0.021)%,(0.451±0.032)%,and CD4/CD8 levels were(0.92±1.02).In control group,CD4 and CD8 levels were(0.465±0.141)%,(0.347±0.036)%,and CD4/CD8 levels were(1.32±0.22).The corresponding indexes of the two groups were compared,and the difference was statistically significant(all P<0.05).Conclusion:The expression of vitamin D in lung cancer patients is significantly down-regulated,and supplementation and adjustment of vitamin D levels in the body may help regulate the expression of immune cells,which provides a new idea for the treatment of lung cancer patients with low levels of vitamin D(improve
