不同器械预备根管治疗隐裂性牙髓炎的效果分析

摘　　要：目的:探究隐裂性牙髓炎治疗中不同器械预备根管临床应用效果。方法:选取2020年1月~2022年1月92例患者作为研究对象,随机分为2组,各46例,应用不锈钢K锉预备根管表示为K file组,应用Protaper机用镍钛锉预备根管表示为Protaper组。通过2组临床疗效、约诊间痛、填充效果、牙周指数、牙齿功能、不良事件、生活质量等观察比较不同器械预备根管应用效果。结果:Protaper组临床治疗总有效率为95.65%,高于Kfile组的82.61%(P<0.05);根管治疗约诊间痛发生率8.70%,低于K file组的26.09%(P<0.05);根管适充率95.65%,高于K file组的82.61%(P<0.05);牙周探诊深度(2.25±0.34)mm、牙龈出血指数(1.35±0.41)分、菌斑指数(1.32±0.44)分,浅/低于K file组的(2.52±0.17)mm、(1.66±0.21)分、(1.67±0.23)分(P<0.05);咬合力(58.29±3.54)kg、咀嚼效率(84.58±3.44)%、言语功能(9.54±1.07)分,高于K file组(P<0.05);不良事件发生率4.35%,低于K file组的17.39%(P<0.05);口腔健康影响程度评分(6.05±2.22)分,低于K file组的(8.56±2.88)分(P<0.05)。结论:隐裂性牙髓炎治疗中Protaper机用镍钛锉预备根管应用效果较不锈钢K锉预备根管好,对治疗有效性、安全性提升存在积极影响。Objective:To investigate the clinical effect of root canal preparation with different instruments in the treatment of cracked pulpitis.Methods:92 patients were selected as the study objects.The study objects were collected from January 2020 to January 2022.They were randomly divided into two groups,46 cases in each group.One group was treated with stainless steel K file to prepare root canal,which was represented as K file group,and the other group was treated with nickel titanium file to prepare root canal,which was represented as Protaper group.The clinical efficacy,pain between appointments,filling effect,periodontal index,tooth function,adverse events and quality of life of the two groups were observed and compared to evaluate the application effect of different instruments in root canal preparation.Results:The total effective rate of clinical treatment in the Protoper group was 95.65%,which was higher than that in the K-file group(82.61%)(P<0.05);The incidence of pain between appointments in root canal therapy was 8.70%,lower than 26.09%in K file group(P<0.05);The root canal filling rate was 95.65%,higher than 82.61%in K file group(P<0.05);The depth of periodontal probing was(2.25±0.34)mm,the gingival bleeding index was(1.35±0.41),the plaque index was(1.32±0.44),and the shallow/lower K file group was(2.52±0.17)mm,(1.66±0.21),and(1.67±0.23)(P<0.05);Bite force(58.29±3.54)kg,masticatory efficiency(84.58±3.44)%,and speech function(9.54±1.07)scores were higher than those in K file group(P<0.05);The incidence of adverse events was 4.35%,lower than 17.39%in K file group(P<0.05);Oral health impact score(6.05±2.22)was lower than that of K file group(8.56±2.88)(P<0.05).Conclusion:Protaper machine nickel titanium file is better than stainless steel K file in root canal preparation in the treatment of cracked pulpitis,which has a positive impact on the improvement of treatment effectiveness and safety.
