HR-HPV阴性的ASC-US患者发生宫颈高级别上皮内病变相关因素分析

摘　　要：目的分析高危型人乳头瘤病毒(HR-HPV)阴性、宫颈细胞学为不能明确意义的非典型鳞状细胞(ASC-US)的患者发生宫颈高级别鳞状上皮内病变(HSIL)及以上病变的相关因素。方法选取2021年1月~2022年5月就诊于潍坊医学院附属医院宫颈病变门诊的360例患者为研究对象,患者宫颈液基细胞学为ASC-US,HR-HPV为阴性,均行阴道镜检查+宫颈活检术,并统计分析其临床资料。结果360例患者中HSIL+24例(6.67%),低级别鳞状上皮内病变(LSIL)147例(40.83%),慢性炎症189例(52.5%)。单因素分析结果显示,患者是否绝经、阴道镜评估结果在≥HSIL组与≤LSIL组之间差异均有统计学意义(P<0.05)。多因素分析结果显示,绝经、阴道镜评估结果均对HR-HPV阴性的ASC-US患者发生HSIL及以上病变的影响具有统计学意义(P<0.05)。结论HR-HPV阴性的ASC-US患者中阴道镜检查评估结果≥HSIL的患者发生HSIL及以上病变的风险增加,绝经的患者发生HSIL及以上病变的风险较低。Objective To explore the related factors of high-grade cervical squamous intraepitheliallesion(HSIL)and above in patients with high-risk human papillomavirus(HR-HPV)negative atypical squamous cells(ASC-US)with uncertain cervical cytology.Methods A total of 360 patients who were admitted to the outpatient clinic of cervical lesions in the Affiliated Hospital of Weifang Medical University from January 2021 to May 2022 were selected for the study.The cervical fluid-based cytology of the patients was ASC-US,HR-HPVwas negative,and all patients were examined by colposcopy and cervical biopsy.Results There were 24 cases of HSIL+(6.67%),147 cases of low-grade squamous intraepithelial lesion(LSIL)(40.83%)and 189 cases of chronic inflammation(52.5%).Univariate analysis showed that there were significant differences in menopausal status and colposcopy evaluation between≥HSIL group and≤LSIL group.The results of multivariate analysis showed that menopause and colposcopy had significant effects on the occurrence of HR-HPV and above lesions in ASC-US patients with negative HSIL.Conclusion InHR-HPV-negative ASC-US patients,patients withcolposcopy≥HSIL had an increased risk of developing HSIL and above,while menopausal patients had a lower risk of developing HSIL and above.
