摘　　要：目的探讨神经导航系统对急症高血压脑内出血手术中的应用及总结操作技巧。方法2021年10月~2023年3月,诸城市人民医院收治的因急诊脑出血病例42例,行神经导航引导下颅内血肿钻孔引流术手术病例20例(导航组),行非导航颅内钻孔引流术22例(非导航组)比较两组患者早期血肿清除量、术前斯堪的纳维亚卒中预后评分及随访评分量表(SSS)、术前格拉斯哥昏迷评分(GCS)、术后3个月格拉斯哥预后评分(GOS评分)、再次出血率、术后并发症发生率的高低。结果导航组术后3个月SSS评分高、早期血肿清除率高、住院天数短,有统计学意义,术后再出血、死亡率低,总并发症率差异有统计学意义。结论行神经导航系统引导下手术,操作精准,在改善神经功能恢复、缩短康复时间、住院日、减少再出血并发症等方面有一定优势。Objective To explore the application of magnetic navigation system in emergency hypertensive intracerebral hemorrhage surgery and summarize techniques.Methods From October 2021 to March 2023,42 cases of emergency intracerebral hemorrhage were admitted to the People's Hospital of Zhucheng,including 20 cases who underwent magnetic navigation guided intracranial hematoma drilling and drainage surgery(navigation group)and 22 cases who underwent non navigation intracranial drilling and drainage surgery(non navigation group).The early hematoma clearance,preoperative Scandinavian Stroke Prognostic Score and Follow up Score(SSS),preoperative Glasgow Coma Scale(GCS),and preoperative Glasgow Coma Scale(GCS)were compared between the two groups the Glasgow Outcome Scale(GOS score),the rate of recurrent bleeding,and the incidence of postoperative complications at 3 months after surgery.Results The navigation group had a high SSS score,high early hematoma clearance rate,and short hospitalization days at 3 months after surgery,which was statistically significant.There was a significant difference in postoperative rebleeding,low mortality rate,and overall complication rate.Conclusion Neuronavigation system guided surgery has certain advantages in improving neurological function recovery,shortening rehabilitation time,hospitalization days,and reducing complications of recurrent bleeding.
