斜形分根法在下颌水平阻生第三磨牙拔除术中的应用

摘　　要：目的 观察斜形分根法在下颌水平阻生第三磨牙拔除术中的临床效果。方法 行下颌水平阻生第三磨牙拔除术患者80例,随机分成两组,每组40例。试验组采用斜形分根法,对照组采用T形分冠法。比较两组手术时间、阻生牙舌侧牙槽骨折裂情况、吞咽疼痛、术后肿胀、术后VAS疼痛评分。结果 试验组手术时间为(16.48±0.93) min,短于对照组的(21.05±1.50) min(P<0.05)。试验组舌侧牙槽骨折裂、吞咽疼痛发生率低于对照组(P<0.05)。试验组术后24、72 h和术后7 d肿胀较对照组稍小(P>0.05)。术后4 h,试验组VAS疼痛评分较对照组低(P<0.05),术后24、48 h两组VAS疼痛评分比较无统计学差异(P>0.05)。结论 在下颌水平阻生第三磨牙拔除术中,斜形分根法可以较好地解决根部阻力较大问题,缩短手术时间,保护舌侧骨壁,减轻术后疼痛。Objective To observe the clinical efficacy of oblique root division method in the extraction of impacted mandibular horizontal third molar.Methods Eighty patients undergoing the extraction of impacted mandibular horizontal third molar were randomly divided into two groups with 40cases each.Group A was treated with oblique root division method and group B was treated with T-shaped root division method.The operation time,lingual alveolar fracture and fissure of impacted teeth,swallow pain,swelling and VAS pain score after surgery were compared between the two groups.Results The operation time was shorter in group A than that in group B[(16.48±0.93)minutes vs.(21.05±1.50)minutes](P<0.05).The incidences of lingual alveolar fracture fissure and swallow pain after surgery were lower in group A than those in group B(P<0.05).The swelling of group A was slightly smaller than that of group B at 24hours,72hours and 7days after surgery(P>0.05).The VAS pain score was lower in group A than that in group B at 4hours after surgery(P<0.05).There was no significant difference in VAS pain score between the two groups at 24and 48hours after surgery(P>0.05).Conclusion In the extraction of impacted mandibular horizontal third molar,oblique root division method can better solve the problem of greater root resistance,shorten the operation time,protect the lingual bone wall,and reduce postoperative pain.
