血清叶酸、维生素B_(12)及同型半胱氨酸水平与男性慢性精神分裂症患者认识功能的关系

摘　　要：目的探讨血清叶酸、维生素B_(12)(Vitamin B_(12),VB_(12))、同型半胱氨酸水平(Homocysteine levels,Hcy)与男性精神分裂症患者的认知功能相关性。方法选取上海市民政X精神卫生中心住院的男性精神分裂症患者101例,和同期上海健康对照组51例,测定叶酸、VB_(12)、Hcy与精神分裂症患者的认知功能相关性。结果研究组和对照组年龄、文化程度、并发高血压或糖尿病患病率及文化程度差异无统计学意义(P>0.05);研究组患者的即刻记忆、视觉广度、言语功能、注意、延时记忆、神经心理状态评定量表(Neuropsychological State Assessment Scale,RBANS)总分、VB_(12)、Hcy水平及叶酸水平差异有统计学意义(P<0.05);研究组VB_(12)、叶酸水平均低于对照组,且Hcy水平高于对照组,差异有统计学意义(P<0.05);研究组叶酸水平与阴性症状呈负相关,VB_(12)水平与患者年龄、病程呈负相关,与阳性症状呈正相关,Hcy水平与视觉广度、叶酸及VB_(12)均呈负相关。结论男性慢性精神分裂症患者血清HCY升高、VB_(12)偏低,与精神分裂症患者认知功能存在相关性。Objective Explore the correlation between serum folate,VB_(12),and homocysteine levels and cognitive function in patients with schizophrenia.Methods 101 patients with schizophrenia admitted to the Shanghai Civil Affairs X Mental Health Center and 51 healthy controls were selected to determine the correlation between folic acid,VB_(12),Hcy,and cognitive function in patients with schizophrenia during the same period.Results There was no significant difference between the study group and the control group in age,education level,prevalence of hypertension or diabetes and education level(P>0.05).There were statistically significant differences in immediate memory,visual span,speech function,attention,delayed memory,total RBANS score,VB_(12),Hcy levels,and folate levels among the study group patients(P<0.05).The levels of VB_(12)and folic acid in the study group were lower than those in the control group,and there was a statistically significant difference in Hcy levels compared to the control group(P<0.05).The study group showed a negative correlation between folic acid levels and negative symptoms,a negative correlation between VB_(12)levels and patient age and course of disease,a positive correlation between positive symptoms,and a negative correlation between Hcy levels and visual span,folic acid,and VB_(12).Conclusion The elevated serum HCY and low serum VB_(12)levels in male patients with chronic schizophrenia are correlated with cognitive function.
