基于同伴互助的团体认知行为治疗对不同住院时长的男性慢性精神分裂症患者的效果

摘　　要：目的探讨不同住院时间男性慢性精神分裂症患者进行基于同伴互助的团体认知行为治疗后阴性症状、社会功能及认知功能障碍改善程度之间的差异。方法按照住院时间,分为≥3年和<3年的团体治疗组和药物治疗组,在治疗基线、第12周以及20周,分别采用阳性与阴性症状量表(PANSS)中的阴性症状量表、社会功能评定量表(SSFPI)、自尊量表(SES)、霍普金斯语言学习测验(HVLT)、范畴流畅测验(CFAN)进行临床评估。结果治疗第12周,无论住院时间长短,团体治疗组HVLT、CFAN、SES、SSFPI均高于药物治疗组,PANSS低于药物治疗组;第20周,住院时间≥3年,HVLT、CFAN、SES均高于药物治疗组;住院时间<3年,CFAN及SES高于药物治疗组,PANSS低于药物治疗组(均P<0.05)。结论本研究表明不论住院时间长短,基于同伴互助的团体认知行为治疗干预后对男性慢性精神分裂症患者的阴性症状、社会功能及认知功能障碍疗效更佳,且效果持久,值得临床广泛应用。Objective To explore the improvement differences in negative symptoms,social function,and cognitive dysfunction after peer support group cognitive behavioral therapy(CBT)for male chronic schizophrenic patients with different lengths of hospitalization.Methods According to lengths of hospitalization,all the subjects were divided into 4 groups(<3years and group treatment,<3years and medicine treatment,≥3 years group treatment,≥3 years and medicine treatment).The negative symptom scale(PANSS),Social Functioning Scale for Hospitalized Psychiatric Patients(SSFPI),Self-Esteem Scale(SES),Hopkins Verbal Learning Test(HVLT),and the Category Fluency Test(CFAN)were used to assess all the subjects at baseline,after 12 weeks and 20 weeks of treatment.Results After 12 weeks of treatment,regardless of the length of hospitalization,HVLT,CFAN,SES and SSFPI were higher than in drug therapy group,PANSS negative symptoms scale was lower than the drug therapy group.At 20 weeks,for hospitalized time more than 3 years,HVLT,CFAN,and SES were higher than in drug treatment group,for hospitalized time within 3 years,CFAN and SES were higher than drug therapy group,PANSS was lower than the drug therapy group(P<0.05).Conclusion peer support GCBT were effective in negative symptoms,social functioning and cognitive dysfunction for male patients with chronic schizophrenia,regardless of the length of hospitalization.
