芍药汤加减联合西医治疗对伴焦虑、抑郁的溃疡性结肠炎患者的影响

摘　　要：目的探讨芍药汤加减联合西医治疗对溃疡性结肠炎(UC)伴焦虑、抑郁患者的疗效及对其血清炎症因子的影响。方法UC伴焦虑抑郁患者110例,随机分为对照组55例与试验组55例;对照组予以帕罗西汀及美沙拉嗪治疗,试验组在对照组基础上予以芍药汤加减治疗,两组均治疗8周。观察两组临床疗效和不良反应,比较两组治疗前和治疗8周临床症状评分、焦虑自评量表(SAS)、抑郁自评量表(SDS)、炎症因子包括肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)和白细胞介素-8(IL-8)。结果相比对照组(79.63%),试验组总有效率(94.34%)明显较高(P<0.05)。治疗后,试验组临床症状评分、SAS评分、SDS评分显著低于对照组(P<0.05)。治疗后,试验组血清TNF-α、IL-6和IL-8水平显著低于对照组(P<0.05)。两组不良反应发生率无统计学差异(P>0.05)。结论对于UC伴焦虑抑郁患者,芍药汤加减联合西医治疗能够更好地缓解临床症状,减轻焦虑抑郁情绪和炎症反应,提高临床疗效。Objective To explore the curative effect of modified Shaoyao decoction combined with western medicine on ulcerative colitis(UC)with anxiety and depression and its influence on serum inflammatory factors.Methods A total of 110 UC patients with anxiety and depression were randomly divided into control group(55 cases)and treatment group(55 cases).The control group was treated with paroxetine and Mesalazine,and the experimental group was treated with Shaoyao Decoction on the basis of the control group.Both groups were treated for 8 weeks.The clinical efficacy and adverse reactions of the two groups were observed.The clinical symptom scores,self-rating anxiety scale(SAS),self-rating depression scale(SDS),inflammatory factors[tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)and IL-8]and immune function indexes were compared between the two groups before and 8 weeks after treatment.Results Compared with the control group(79.63%),the total effective rate of the experimental group(94.34%)was significantly improved(P<0.05).After treatment,the clinical symptom score,SAS score and SDS score of the experimental group were significantly lower than those of the control group(P<0.05).After treatment,the serum levels of TNF-α,IL-6 and IL-8 in the experimental group were significantly lower than those in the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion For UC patients with anxiety and depression,Shaoyao Decoction combined with Deanxit and Mesalazine can better alleviate clinical symptoms,reduce anxiety and depression and inflammatory response,and improve clinical efficacy.
