UDPd/Scr比值对口腔种植修复患者术后种植体周围炎发生的预测分析

摘　　要：目的探讨尿脱氧吡啶酚(UDPd)/肌酐(Scr)比值对口腔种植修复患者术后发生种植体周围炎的预测价值。方法本研究为一项前瞻性研究,纳入2020年1月至2022年12月商丘市第一人民医院给予口腔种植修复术治疗患者为研究对象,于术前、术后7 d、术后15 d检测患者UDPd/Scr比值。患者术后随访6个月,统计患者术后种植体周围炎发生情况并依据结果将患者分为发生组、未发生组。比较两组患者术后2 d、7 d、15 d时UDPd/Scr,并绘制受试者工作特征(ROC)曲线,分析UDPd/Scr对口腔种植修复患者术后发生种植体周围炎的预测价值。结果随访期间,因患者自身原因230例患者中途退出3例,完成随访工作共227例。227例口腔种植修复患者术后发生种植体周围炎42例,占比18.50%。发生组与未发生组患者术后2 d、7 d、15 d时UDPd/Scr逐渐升高,但发生组术后2 d、7 d、15 d时UDPd/Scr均高于未发生组,差异有统计学意义(P<0.05)。绘制ROC曲线,结果显示,术后2 d、7 d、15 d时UDPd/Scr预测口腔种植修复患者术后发生种植体周围炎的曲线下面积(AUC)均≥0.7,均有一定的预测价值。结论口腔种植修复患者术后UDPd/Scr比值高与术后种植体周围炎发生有关,术后早期检测UDPd/Scr有助于预测患者术后种植体周围炎发生风险。【Objective】To explore the predictive value of the ratio of urinary deoxypyridinoline(UDPd)/serum creatinine(Scr)for the occurrence of peri-implant inflammation in oral implant repair patients after surgery.【Methods】This study was a prospective cohort study.The patients who were treated with dental implant repair in the hospital from January 2020 to December 2022 were included in the study.The UDPd/Scr ratio of patients was measured before operation,7 days after operation,and 15 days after operation.The patient was followed up for 6 months after surgery,and the incidence of peri-implant inflammation was statistically analyzed.Based on the results,the patient was divided into an occurrence group and a non-occurrence group.The UDPd/Scr was compared between the two groups on the 2nd,7th,and 15th day after operation,and the receiver operating characteristic(ROC)curve was drawn to analyze the predictive value of UDPd/Scr for peri-implant inflammation after oral implant repair.【Results】During the follow-up period,3 out of 230 patients withdrew due to their own reasons,and a total of 227 patients completed the follow-up work.Forty-two out of 227 oral implant repair patients developed peri-implant inflammation after surgery,accounting for 18.50%.On the 2nd,7th,and 15th day after surgery,the UDPd/Scr gradually increased in the occurrence group and non-occurrence group patients,but on the 2nd,7th,and 15th day after surgery,the UDPd/Scr in the occurrence group was higher than that in the non-occurrence group,with a statistical significant difference(P<0.05).The ROC curve was drawn,and the results showed that the area under the curve(AUC)of UDPd/Scr predicting peri-implant inflammation in patients with oral implant repair on the 2nd,7th,and 15th day after surgery was≥0.7,which had certain predictive value.【Conclusion】The high postoperative UDPd/Scr ratio in patients with oral implant repair is related to the occurrence of peri-implant inflammation.Early detection of UDPd/Scr after surgery can help predict th
