摘  要:目的研究天珠散对缺血性脑卒中大鼠神经功能损伤的保护作用机制。方法选取90只雄性SD大鼠,随机取10只作为假手术组,其余大鼠参照Longa线栓法制备大脑中动脉栓塞脑缺血再灌注模型。将造模成功后的大鼠随机分为模型组、尼莫地平组及天珠散低、中、高剂量组,每组16只。造模12 h后,尼莫地平组给予尼莫地平20 mg/kg灌胃,天珠散低、中、高剂量组分别给予天珠散300 mg/kg、600 mg/kg、1200 mg/kg灌胃,假手术组和模型组给予蒸馏水灌胃,均1次/d,连续10 d。分别于灌胃第1天、第3天、第5天、第7天、第10天对各组大鼠进行Longa评分及转棒实验(记录转棒停留时间);末次灌胃结束后2 h,MRI扫描成像和TTC染色测定大鼠脑梗死体积,HE染色观察脑组织病理形态,Western blot法检测脑组织中Wnt3a、微管相关蛋白2(MAP-2)蛋白表达情况。结果与假手术组比较,模型组大鼠出现明显神经功能缺损和梗死灶,转棒停留时间明显缩短(P<0.05),脑组织中Wnt3a蛋白相对表达量明显升高(P<0.05),脑组织中MAP-2蛋白相对表达量明显降低(P<0.05);缺血侧海马CA1区锥体细胞排列稀疏紊乱,神经元肿胀或萎缩脱失,空泡样改变。与模型组比较,各给药组大鼠Longa评分、脑梗死体积比、脑组织中Wnt3a蛋白相对表达量均明显降低(P均<0.05),转棒停留时间均明显延长(P均<0.05),脑组织中MAP-2蛋白相对表达量均明显升高(P均<0.05);缺血侧脑组织病理损伤有所减轻,神经细胞排列规则,空泡样变和坏死数相对减少,其中尼莫地平组和天珠散高剂量组改善更明显。结论天珠散可能通过调控Wnt信号通路关键效应分子Wnt3a及MAP-2蛋白的表达,促进神经细胞增殖分化和突触重塑,发挥对脑缺血的神经保护作用。Objective It is to study the mechanism of protective effect of Tianzhu Powder on the neurological impairment of rats with ischemic stroke.Methods Ninety male SD rats were selected,in which 10 rats were randomly selected as sham operation group,and the rest of the rats were used to establish cerebral ischemia-reperfusion models of middle cerebral artery embolism by Longa wire embolism method.The successfully modeled rats were randomly divided into model group,nimodipine group,and low,medium and high dose groups of Tianzhu powder,with 16 rats in each group.At 12 h after modeling,the nimodipine group was given nimodipine 20 mg/kg by gavage,the low,medium and high dose groups of Tianzhu powder were given Tianzhu powder 300 mg/kg,600 mg/kg and 1200 mg/kg by gavage,respectively,and the sham operation group and model group were given distilled water by gavage,all once daily,continuously treated for 10 days.On the 1st,3rd,5th,7th and 10th day of gavage,the rats in each group were evaluated by Longa scoring and rotarod test(the residence time was recorded);at 2h after the end of the last gavage,the volume of cerebral infarction in rats was detected by MRI scanning and TTC staining,the pathological morphology of brain tissues was observed by HE staining,and the expressions of Wnt3a and microtubule-associated protein 2(MAP-2)proteins in brain tissues were detected by Western blot method.Results Compared with the sham operation group,the rats in the model group had obvious neurological deficits and infarct foci,their residence time was significantly shortened(P<0.05),the relative expression of Wnt3a protein in cerebral tissue was significantly increased(P<0.05),and the relative expression of MAP-2 protein in cerebral tissue was significantly decreased(P<0.05);the arrangement of pyramidal cells in hippocampal CA1 area of the ischemic sides was sparse and disordered,with neuronal swollen or atrophic deletion,vacuole-like changes.Compared with the model group,the Longa score,cerebral infarction volume ratio,relative expression
