针刺联合太极云手中医特色功能训练对不完全脊髓损伤患者神经功能康复的影响

摘　　要：目的评价针刺联合太极云手中医特色功能训练治疗不完全脊髓损伤的效果,以优化诊疗方案。方法选取四川省骨科医院康复科2017年10月—2020年10月收治的40例不完全脊髓损伤患者为研究对象,采取随机法将患者分为对照组和观察组各20例,对照组给予常规康复治疗(物理治疗+常规训练),观察组在对照组干预基础上给予太极云手中医特色功能训练联合针刺治疗,干预3个月。观察2组患者治疗前后脊髓损伤分级(ASIA神经功能分级)、肌力[下肢运动评分(LEMS)]、平衡能力(Berg平衡量表评分)与运动耐力(起立-步行计时、10 m步行时间)及步行功能[步行功能分级(Holden)]。结果观察组治疗后ASIA神经功能分级和Holden分级改善情况均明显优于对照组(P均<0.05),LEMS评分及Berg平衡量表总分均明显高于对照组(P均<0.05),起立-步行计时、10 m步行时间均明显短于对照组(P均<0.05)。结论针刺联合太极云手中医特色功能训练可促进不完全脊髓损伤患者神经功能的恢复,提高患者平衡能力与独立行走能力。Objective It is to evaluate the clinical effect of Taiji Yunshou traditional Chinese medicine characteristic function training combined with acupuncture on neurological function rehabilitation of patients with incomplete spinal cord injury(SCI).Methods Forty patients with incomplete SCI treated at department of rehabilitation of Sichuan Province Orthopedic Hospital from October 2017 and October 2020 were selected as the research subjects.According to the random method,the research subjects were divided into control group and observation group,with 20 cases in each group.The control group was given conventional rehabilitation treatment(physical therapy+conventional training),the observation group was additionally given Taiji Yunshou traditional Chinese medicine characteristic function training combined with acupuncture based on the control group.Both groups were treated for 3 months.The spinal cord injury classification(ASIA neurological function classification),muscle strength[lower extremity movement score(LEMS)],balance ability(Berg balance scale score)and exercise endurance(get-up-and-walk timing,10 m walking time)and walking function[walking function classification(Holden)]before and after the treatment of patients in the 2 groups were observed.Results The improvement of ASIA neurological function classification and Holden classification of the observation group after treatment were significantly better than those of the control group(all P<0.05),the LEMS scores and total scores of Berg Balance Scale were significantly higher than those of the control group(all P<0.05),and the get-up-and-walk timings and the 10 m walking time were significantly shorter than those of the control group(all P<0.05).Conclusion Taiji Yunshou traditional Chinese medicine characteristic function training combined with acupuncture can promote neurological recovery and improve balance ability and independent walking ability in patients with incomplete spinal cord injuries.
