超声弹性成像评价柴当方治疗重度非酒精性单纯性脂肪肝的疗效研究

摘　　要：目的 初步观察柴当方对重度非酒精性单纯性脂肪肝的疗效和安全性。方法 采用自身前后对照的研究方法观察柴当方治疗重度非酒精性单纯性脂肪肝的疗效。运用柴当方干预26例重度非酒精性单纯性脂肪肝患者,每日1剂,1剂中药煎成2袋,每袋150ml,每次1袋,每日2次,温服,治疗14天。应用超声弹性成像检测肝脂肪变性程度,治疗前后各检测1次,观察治疗前后肝脂肪变性程度的变化,并观察不良反应。结果 治疗后与治疗前比较,患者肝脂肪变性程度变小,差异均具有统计学意义(P<0.05),未出现不良反应。痊愈4例,显效5例,有效12例,无效5例,总有效率80.8%。结论 柴当方可以减小脂肪肝患者肝脏脂肪变性程度,具有直接治疗非酒精性单纯性脂肪肝的作用,具有良好的安全性。服用柴当方14天即可显示良好效果,表明柴当方治疗脂肪肝快速有效。柴当方可以治疗非酒精性单纯性脂肪肝,具有广阔的应用前景。柴当方由柴胡桂枝干姜汤合当归芍药散组成,为经方合方的应用和研究提供了新的思路。Objective To investigate the efficacy and safety of Chaidangfang Formula in the treatment of severe non alcoholic simple fatty liver disease.Methods A self controlled study method was used to observe the therapeutic effect of Chaidangfang Formula on severe non alcoholic simple fatty liver.Using Chaidangfang Formula to intervene in 26 patients with severe non-alcoholic simple fatty liver,one dose per day was decocted into two bags of traditional Chinese medicine,each bag containing 150ml,one bag per time,twice a day,taken warm for 14 days.Apply ultrasound elastography to detect the degree of liver steatosis,once before and after treatment,observe the changes in the degree of liver steatosis before and after treatment,and observe adverse reactions.Results After treatment,compared with before treatment,the degree of liver steatosis in patients decreased,and the differences were statistically significant(P<0.05).No adverse reactions occurred.4 cases were cured,5 cases were significantly improved,12 cases were effective,and 5 cases were ineffective,with a total effective rate of 80.8%.Conclusion Chaidangfang Formula can reduce the degree of liver steatosis in patients with fatty liver disease,and has a direct therapeutic effect on non alcoholic simple fatty liver disease,with good safety.Taking Chaidangfang Formula for 14 days can show good results,indicating that Chaidangfang Formula is fast and effective in treating fatty liver.Chaidangfang Formula can treat non alcoholic simple fatty liver disease and has broad application prospects.Chaidangfang Formula is composed of Chai hu gui zhi gan jiang tang Formula and Dang gui shao yao san Formula,providing new ideas for the application and research of classic formula combination.
