MRI T_(2)^(*)技术评估地中海贫血患者心脏、肝脏及胰腺铁负荷的相关性研究

摘　　要：目的:基于1.5T磁共振成像(MRI)T_(2)^(*)技术探讨地中海贫血(TM)患者心脏、肝脏及胰腺铁负荷的相关性。方法:收集163例TM患者不同时期MRI T_(2)^(*)图像数据(每次间隔6个月),并按照不同患者首次检查时间,分成间隔6个月、12个月、18个月及24个月进行亚组分析。采用CMRtools软件测量所有患者心脏T_(2)^(*)(H-T_(2)^(*))、肝脏T_(2)^(*)(L-T_(2)^(*))值,并探究两者之间的关系。另收集50例TM患者H-T_(2)^(*)、L-T_(2)^(*)及胰腺T_(2)^(*)(P-T_(2)^(*))数据,探讨P-T_(2)^(*)与H-T_(2)^(*)之间、P-T_(2)^(*)与L-T_(2)^(*)之间的相关性。结果:首次检查组、6个月组及12个月组的H-T_(2)^(*)与L-T_(2)^(*)之间(r=0.431、0.492、0.430,P均<0.001)均呈中度正相关。18个月组及24个月组的H-T_(2)^(*)与L-T_(2)^(*)之间(r=0.326、0.367,P=0.031、0.046)均呈低度正相关。P-T_(2)^(*)与H-T_(2)^(*)、L-T_(2)^(*)均呈中度正相关(r=0.455、0.586,P=0.001,P<0.001)。结论:TM患者H-T_(2)^(*)、L-T_(2)^(*)及P-TT_(2)^(*)之间相关程度不明显,相互反映彼此铁负荷的可靠程度较低。采用MRI T_(2)^(*)技术评估TM患者铁负荷时应进行较全面的脏器铁评估。Objective:To investigate the correlation of iron load in heart,liver and pancreas in thalassemia(TM)patients based on T_(2)^(*)WI on 1.5T MRI.Methods:T_(2)^(*)images of 163 TM patients in different periods(6 months intervals)were collected and divided into subgroup analysis at intervals of 6,12,18 and 24 months according to the time of the first examination.Cardiac T_(2)^(*)(H-T_(2)^(*))and liver T_(2)^(*)(L-T_(2)^(*))values were measured using CMRtools software,and the correlation was performed.In addition,H-T_(2)^(*),L-T_(2)^(*)and pancreatic T_(2)^(*)(P-T_(2)^(*))of 50 TM patients were correlated.Results:There were moderate positive correlation between H-T_(2)^(*)and L-T_(2)^(*)in the first,6-month,and 12th-month groups(r=0.431,0.492,and 0.430,respectively,P<0.001).There was a low-degree positive correlation between H-T_(2)^(*)and L-T_(2)^(*)in the 18th and 24th-month groups(r=0.326,0.367,P=0.031,0.046).P-T_(2)^(*)showed a moderate positive correlation with H-T_(2)^(*)and L-T_(2)^(*)(r=0.455,0.586,P=0.001,<0.001).Conclusion:The correlation of H-T_(2)^(*),L-T_(2)^(*)and P-T_(2)^(*)among TM patients is indeterminate,and less reliable to reflect each other's iron load.A more comprehensive organ iron assessment should be performed in TM patients when assessing iron load by MRI T_(2)^(*)technology.
