摘  要:目的探讨核心稳定性训练对痉挛型脑瘫患者核心肌群形态、粗大运动功能及平衡功能的影响。方法纳入2019年1月-2021年12月因痉挛型脑瘫就诊于兰州大学第二医院儿童康复医学科门诊的患者42例,随机分为常规组(20例)和训练组(22例)。2组患者均采用常规康复治疗,观察组在常规康复治疗的基础上增加核心稳定性训练。分别于治疗前、治疗3个月、治疗6个月记录粗大运动功能测试量表及Berg平衡量表得分,超声检查肌骨,观察腹直肌、腹外斜肌和竖脊肌厚度及多裂肌横截面积的变化。结果组内比较,治疗3个月时,常规组仅右侧竖脊肌厚度较治疗前差异有统计学意义(P<0.05),而训练组腹直肌厚度、腹外斜肌厚度、竖脊肌厚度和多裂肌水平面面积,以及粗大运动功能测试量表和Berg平衡量表得分,与治疗前比较,差异均具有统计学意义(P<0.05)。组间比较,训练组腹直肌厚度、腹外斜肌厚度、竖脊肌厚度和多裂肌水平面面积,在治疗3、6个月时,均明显高于常规组,差异有统计学意义(P<0.05);与训练组比较,常规组粗大运动功能测试量表和Berg平衡量表得分治疗6个月时差异才具有统计学意义(P<0.05)。结论核心稳定性训练较常规训练,更有利于提高痉挛型脑瘫患者的核心稳定性,改善其粗大运动功能能力。Objective To examine how core stability training impacts the morphology of core muscles,gross motor function and balance in children with spastic cerebral palsy.Methods From January 2019 to December 2021,42 patients with spastic cerebral palsy were admitted to Lanzhou University Second Hospital;those with the disorder were randomly assigned into two groups:a control group(20 cases)and training group(22 cases).Both received routine rehabilitation treatment,while additional core stability training was given to the observation group.Muscle and skeletal ultrasonography were performed to determine changes in rectus abdominis,lateral oblique,erector spinal muscle thickness and cross-sectional area of the multifidus muscle.The gross motor function measurement scale and Berg balance scale scores were recorded at three months and six months after treatment,respectively,for both groups.Results In the intra-group comparison,at 3 months of treatment,only the thickness of the right erector spinal muscle in the control group was significantly different from that before treatment(P<0.05),while the training group had significant differences inrectus abdominis thickness,external oblique muscle thickness,erector spinal muscle thickness and cross-sectional area of multifidus muscle,as well as in the scores of the gross motor function test scale and the Berg balance scale compared with thsoe before treatment(P<0.05).Between group comparisons,the thickness of rectus abdominis muscle,external oblique muscle thickness,erector spinus muscle thickness and crosssectional area of multifidus muscle were significantly higher than those of the control group at 3 and 6 months of treatment,and the differences were statistically significant(P<0.05);compared with the training group,there was a significant difference in the control group with gross motor function test scale and Berg balance scale score after 6 months of treatment(P<0.05).Conclusion Core stability training is more conducive to improving the core stability of children with spastic c
