吴又可运用三承气汤辨治温疫思想探析

摘　　要：三承气汤为《伤寒论》中调胃承气汤、小承气汤、大承气汤的统称,是张仲景治疗阳明腑实证的经典方剂。吴又可推崇张仲景思想,探究三承气汤的经义,结合其对温疫的认识,将三承气汤灵活运用于温疫辨治之中。从病机认识、证候辨识、因证施治以及守正创新等方面,揭示吴又可运用三承气汤辨治温疫思想。指出吴又可治疗温疫的要点在于尽早逐邪,而三承气汤本为逐邪而设;推崇温疫舌象诊断,总结脉象以及特有临床征象;变通古法以合新病,对三承气汤灵活化裁等,拓展了三承气汤的辨治体系,为后世认识与运用三承气汤辨治温疫提供借鉴。Three Chengqi Decoctions are the general name of Tiaowei Chengqi Decoction,Xiao Chengqi Decoction and Da Chengqi Decoction in Treatise on Febrile Diseases,which were classic prescription of ZHANG Zhongjing for treating Yangming fu-viscera excess syndrome.WU Youke praise highly ZHANG Zhongjing's thought,explored the classic meaning of three Chengqi Decoctions,flexible used three Chengqi Decoctions in the diagnosis and treatment of warm epidemic,which combining the understanding of the epidemic.From the aspects of understanding the pathogenesis,syndrome identification,treatment based on symptoms,and innovation in maintaining integrity,to reveal WU Youke's thought of using three Chengqi Decoctions to differentiate and treat warm epidemic,to point out the key to WU Youke's treatment of epidemic fever lies in early elimination of pathogenic factors,and the original intention of three Chengqi Decoctions is to eliminate evil qi.WU Youke praised the diagnosis of warm epidemic tongue symptoms,summarized pulse and unique clinical signs,adapted ancient methods to adapt to new diseases,used three Chengqi Decoctions flexibly.All of these have expanded the differentiation and treatment system of three Chengqi Decoctions,and provide reference for future generations to understand and use the three Chengqi Decoctions for the differentiation and treatment of epidemic diseases.
