解郁安神汤联合碳酸锂对双相情感障碍躁狂发作患者认知功能及生活质量的影响  

Effect of Jieyu Anshen Decoction Combined with Lithium Carbonate on Cognitive Function and Quality of Life in Patients with Bipolar Manic Episode

摘　　要：目的:观察解郁安神汤联合碳酸锂对双相情感障碍躁狂发作患者认知功能及生活质量的影响。方法:选取94例双相情感障碍躁狂发作患者,按随机数字表法分为观察组和对照组各47例。对照组使用碳酸锂治疗,观察组使用解郁安神汤联合碳酸锂治疗,均治疗8周。比较2组治疗前后贝克-拉范森躁狂量表(BRMS)评分、临床总体印象疾病严重度量表(CGI-s-BP)评分、生活质量综合评定问卷(GQOLI-74)评分及认知功能——重复性成套神经心理状态测验(RBANS)评分,并比较2组临床疗效。结果:治疗后,2组BRMS、CGI-s-BP评分均降低(P<0.05),且观察组低于对照组(P<0.05)。观察组总有效率87.23%,高于对照组65.96%(P<0.05)。治疗后,2组GQOLI-74各项评分均升高(P<0.05),且观察组高于对照组(P<0.05)。治疗后,2组各项RBANS评分及总分均升高(P<0.05),且观察组高于对照组(P<0.05)。结论:解郁安神汤联合碳酸锂治疗双相情感障碍躁狂发作的效果优于单纯碳酸锂治疗,且能有效改善患者认知功能和生活质量。Objective:To observe the effect of the therapy of Jieyu Anshen Decoction combined with Lithium Carbonate on cognitive function and quality of life in patients with bipolar manic episode.Methods:A total of 94 patients with bipolar manic episode were selected and divided into the observation group and the control group according to random number table method,with 47 cases in each group.The control group was treated with Lithium Carbonate,and the observation group was additionally treated with Jieyu Anshen Decoction combined with Lithium Carbonate.Both groups were treated for eight weeks.The scores of the Baker-Rafensen Mania Scale(BRMS),Clinical General Impression-Severity-Bipolar disorder(CGI-s-BP),Generic Quality of Life Inventory 74(GQOLI-74) and Repeatable Battery for the Assessment of Neuropsychological Status(RBANS) were compared before and after treatment between the two groups,and the clinical effects were also compared.Results:After treatment,the BRMS and CGI-sBP scores in both groups were decreased(P<0.05),and the above scores in the observation group were lower than those in the control group(P<0.05).The total effective rate was 87.23% in the observation group,higher than that of 65.96% in the control group(P<0.05).After treatment,all scores of GQOLI-74in the two groups were increased(P<0.05),and the scores in the observation group were higher than those in the control group(P<0.05).After treatment,all RBANS scores and total scores were increased in both groups(P<0.05),and those in the observation group were higher than those in the control group(P<0.05).Conclusion:The therapy of Jieyu Anshen Decoction combined with Lithium Carbonate has a better therapeutic effect on patients with bipolar manic episode than simple Lithium Carbonate,which can effectively improve the cognitive function and quality of life of patients.
