疏肝益气消脂方联合多烯磷脂酰胆碱治疗非酒精性脂肪性肝病临床研究

摘　　要：目的:观察疏肝益气消脂方联合多烯磷脂酰胆碱胶囊治疗非酒精性脂肪性肝病(NAFLD)的疗效。方法:选取83例NAFLD患者,按随机数字表法分为治疗组43例与对照组40例。对照组给予多烯磷脂酰胆碱胶囊治疗,治疗组在对照组基础上给予疏肝益气消脂方治疗。治疗2个疗程后,观察2组临床疗效及肝脏B超疗效,比较2组治疗前后肝郁脾虚证症状积分、体质量指数(BMI)、腰围、谷丙转氨酶(ALT)、谷氨酰转肽酶(GGT)、谷草转氨酶(AST)、血脂指标总胆固醇(TC)、三酰甘油(TG)的变化。结果:治疗后,2组肝郁脾虚证症状积分均较治疗前下降(P<0.05),治疗组肝郁脾虚证症状积分低于对照组(P<0.05)。治疗组临床疗效总有效率为93.02%,对照组为75.00%,2组比较,差异有统计学意义(P<0.05)。治疗组肝脏B超总有效率为90.70%,对照组为70.00%,2组比较,差异有统计学意义(P<0.05)。治疗后,2组BMI、腰围均较治疗前下降(P<0.05),治疗组BMI、腰围均低于对照组(P<0.05)。治疗后,2组ALT、GGT、AST、TC、TG水平均较治疗前下降(P<0.05),治疗组上述5项水平均低于对照组(P<0.05)。结论:疏肝益气消脂方联合多烯磷脂酰胆碱胶囊治疗NAFLD疗效确切,可有效减轻患者临床症状,调节BMI及腰围,改善肝功能和血脂指标,进而改善预后。Objective:To observe the curative effect of Shugan Yiqi Xiaozhi Prescription combined with Polyene Phosphatidylcholine Capsules for non-alcoholic fatty liver disease(NAFLD).Methods:A total of83 cases of NAFLD patients were selected and divided into the treatment group and the control group according to the random number table method,with 43 and 40 cases in each group respectively.The control group was treated with Polyene Phosphatidylcholine Capsules,and the treatment group was additionally treated with Shugan Yiqi Xiaozhi Prescription based on the treatment of the control group.After two courses of treatment,the clinical effects and curative effect of liver B ultrasonic examination were observed in the two groups.Before and after treatment,the changes in symptom scores of liver constraint and spleen deficiency syndrome,body mass index(BMI),waist circumference,alanine aminotransferase(ALT),γ-glutamyl transferase(GGT),aspartate aminotransferase(AST),total cholesterol(TC) and triglyceride(TG) were compared between the two groups.Results:After treatment,the symptom scores of liver constraint and spleen deficiency syndrome in the two groups were decreased when compared with those before treatment(P<0.05),and the symptom score of liver constraint and spleen deficiency syndrome in the treatment group was lower than that in the control group(P<0.05).The total clinical effective rate was 93.02% in the treatment group and 75.00% in the control group,the difference being significant(P<0.05).The total effective rate of liver B ultrasonic examination was 90.70% in the treatment group and 70.00% in the control group,the difference being significant(P<0.05).After treatment,BMI and waist circumference in the two groups were decreased when compared with those before treatment(P<0.05),and BMI and waist circumference in the treatment group were lower than those in the control group(P<0.05).After treatment,the levels of ALT,GGT,AST,TC and TG in the two groups were decreased when compared with those before treatment(P<0.05),and the
