IVIM定量参数与软组织肉瘤VEGF、微血管密度表达的相关性研究

摘　　要：目的探讨体素内不相干运动(IVIM)定量参数值与软组织肉瘤(STS)免疫组化指标血管内皮生长因子(VEGF)和微血管密度(MVD)之间的关系。方法选取经术后病理证实的STS患者47例,术前均行常规MRI和IVIM检查,测量并记录肿瘤最长径、标准表观扩散系数(ADCstandard)值、真性扩散系数(D)值、假性扩散系数(D^(*))值及灌注分数(f)值,病理标本染色后获取VEGF得分和MVD值。采用Spearman秩相关分析各影像参数值与VEGF、MVD之间的相关性。结果VEGF得分为(4.04±2.17)分。MVD值为73.78±48.93。VEGF与MVD呈正相关(r=0.446,P=0.002)。肿瘤最长径(7.37±5.15)cm,ADCstandard、D、D*和f值分别为(1.36±0.28)×10^(-3) mm^(2)/s、(1.17±0.26)×10^(-3) mm^(2)/s、(30.46±13.51)×10^(-3) mm^(2)/s和0.26±0.09。D^(*)值与VEGF呈正相关(r=0.450,P=0.002),肿瘤最长径、ADCstandard值、D值、f值与VEGF均无相关性(P>0.05)。D^(*)值和f值与MVD呈正相关(r=0.698、0.423,P<0.05),肿瘤最长径、ADCstandard值、D值与MVD均无相关性(P>0.05)。结论STS的IVIM定量参数D*值和VEGF具有相关性,f值和MVD具有相关性,可用于术前评价STS的血管生成情况。Objective To investigate the correlation between the quantitative parameters of intravoxel incoherent motion(IVIM)with the immunohistochemical indexes of vascular endothelial growth factor(VEGF)and microvessel density(MVD)in soft tissue sarcomas(STS).Methods A total of 47 patients with STS confirmed by pathology after the operation were collected.Routine MRI and IVIM examinations were performed before the operation.The longest diameter,standard apparent diffusion coefficient(ADCstandard)value,D value,D^(*)value,and f value were measured and recorded.VEGF scores and MVD values were obtained after pathological specimen staining.Spearman rank correlation was performed to analyze the correlation between imaging parameters with VEGF and MVD.Results The VEGF score was 4.04±2.17.The MVD value was 73.78±48.93.VEGF was positively correlated with MVD(r=0.446,P=0.002).The longest diameter was(7.37±5.15)cm,and the ADCstandard,D,D*,and f values were(1.36±0.28)×10^(-3) mm^(2)/s,(1.17±0.26)×10^(-3) mm^(2)/s,(30.46±13.51)×10^(-3) mm^(2)/s and 0.26±0.09,respectively.D*value was positively correlated with VEGF(r=0.450,P=0.002),while the longest diameter,ADCstandard value,D value,and f value were not correlated with VEGF(P>0.05).D*and f values were positively correlated with MVD(r=0.698,0.423,P<0.05),while the longest diameter,ADCstandard value,and D value were not correlated with MVD(P>0.05).Conclusion The IVIM quantitative parameters D*values and f values of STS are correlated with VEGF and MVD,which can be used to evaluate the angiogenesis of STS before operation.
