摘 要:目的探讨相位对比磁共振成像(PC-MRI)技术对烟雾病的诊断价值,并分析其在血管重建术后复发中的应用价值。方法选取我院2011年1月至2021年1月80例烟雾病患者作为研究组,另选择同期健康体检者80例作为对照组。两组均行PC-MRI技术检查,比较两组血流动力学参数[颈内动脉、颈外动脉、颞浅动脉、椎动脉血流速度、血流量、血管管腔感兴趣区(ROI)面积]水平,评价各参数对烟雾病的诊断价值,研究组均接受血管重建术治疗,术后随访1年统计复发情况,对比复发与未复发患者颈内动脉、颈外动脉、颞浅动脉、椎动脉血流速度、血流量、血管管腔ROI面积,分析各参数与术后复发的关系。结果研究组颈内动脉血流量小于对照组,颈外动脉、椎动脉血流量大于对照组,颞浅动脉血流量、血流速度、血管管腔ROI面积小于对照组(P<0.05);Spearman相关性分析,颈内动脉血流量、颞浅动脉血流量、血流速度、血管管腔ROI面积与烟雾病发生呈负相关(r=-0.476、-0.461、-0.503、-0.459),颈外动脉、椎动脉血流量与烟雾病发生呈正相关(r=0.422、0.477)(P<0.05);ROC曲线分析,颈内动脉、颈外动脉、椎动脉血流量、颞浅动脉血流量、血流速度、血管管腔ROI面积诊断烟雾病的AUC分别为0.767、0.778、0.828、0.821、0.749、0.748,均>0.7,且以上参数联合诊断的AUC为0.921(95%CI:0.867~0.957),大于各参数单独诊断;复发患者脑血管造影Suzuki分期、血管狭窄程度高于未复发组,颈内动脉血流量、颞浅动脉血流量、血流速度、血管管腔ROI面积小于未复发组,颈外动脉、椎动脉血流量大于未复发组(P<0.05);校正脑血管造影Suzuki分期、血管狭窄程度等其他因素前后,颈内动脉、颈外动脉、椎动脉血流量、颞浅动脉血流量、血流速度、血管管腔ROI面积对术后复发均有重要影响(P<0.05)。结论PC-MRI技术可评估烟雾病患者血流动力学,其血流量Objective To investigate the diagnostic value of phase-contrast magnetic resonance imaging(PC-MRI)technique for smog and to analyze its application value in recurrence after revascularization.Methods Eighty patients with smog disease in our hospital from January 2011 to January 2021 were selected as the study group,and another 80 patients with healthy physical examination in the same period were selected as the control group.Both groups were examined by PC-MRI technique,and the levels of hemodynamic parameters[internal carotid artery,external carotid artery,superficial temporal artery,vertebral artery blood flow velocity,blood flow,area of vascular lumen of interest(ROI)]were compared between the two groups to evaluate the diagnostic value of each parameter for smog disease.External carotid artery,superficial temporal artery,vertebral artery blood flow velocity,blood flow,vascular lumen ROI area,and analyze the relationship between each parameter and postoperative recurrence.Results The blood flow of the internal carotid artery in the study group was smaller than that of the control group,the blood flow of the external carotid artery and vertebral artery was larger than that of the control group,and the blood flow,flow velocity,and vascular lumen ROI area of the superficial temporal artery were smaller than those of the control group(P<0.05).Spearman correlation analysis showed that internal carotid artery blood flow,superficial temporal artery blood flow,blood flow velocity,and vascular lumen ROI area were negatively correlated with the occurrence of smog(r=-0.476,-0.461,-0.503,-0.459),and external carotid artery and vertebral artery blood flow were positively correlated with the occurrence of smog(r=0.422,0.477)(P<0.05).ROC curve analysis,the AU Cs for the diagnosis of smog in the internal carotid artery,external carotid artery,vertebral artery blood flow,superficial temporal artery blood flow,blood flow velocity,and vascular lumen ROI area were 0.767,0.778,0.828,0.821,0.749,and 0.748,respectively,all of which
