摘　　要：目的:探讨折叠游离股前外侧皮瓣在颊癌术后软组织洞穿性缺损修复中的治疗效果。方法:回顾性分析采用折叠游离股前外侧皮瓣修复颊部软组织洞穿性缺损的33例颊部鳞状细胞癌患者的病例资料,男30例,女3例,年龄35~69岁。记录并统计患者基本信息、皮瓣大小,皮瓣脂肪厚度等资料。术后随访2~14个月,术后1、3、6个月定期门诊复查,采用华盛顿大学生存质量量表(UW-QOL)评估患者的生活质量,获取患者的言语、吞咽、咀嚼和外观等信息。采用SPSS 22.0软件对数据进行统计分析。结果:33例患者均无肿瘤复发和转移,皮瓣均完全成活,患者颊部内外侧形态恢复良好,语言、吞咽功能恢复好,无流涎、咬颊症状,下肢负重、行走不受影响。结论:折叠游离股前外侧皮瓣重建颊部洞穿性缺损成功率高,形态功能恢复好,是一种修复颊癌术后软组织洞穿性缺损的可靠方法。Objective:To investigate the therapeutice effects of folded free anterolateral thigh flap for the reconstruction of through and through cheek defect after the resection of buccal carcinoma.Methods:Clinical data of 33 patients underwent the reconstruction of cheek defects with floded free anterolateral thigh flap following removal of squamous carcinoma were analyzed retrospectively.30 males and 3 females,aged 35 to 69 years old were included.The patients were followed up for 2-14 months.1,3 and 6 months after operation,the treatment effects were evaluated with University of Washington Quality of Life(UW-QOL)Questionnaire.The data were statistically analyzed by SPSS 22.0 statistical software.Results:Recurrence and metastasis were not observed in all the cases during the follow-up.All of the 33 folded free flaps survived after operation,the lateral morphology of the cheek was restored well and the mouth opening was normal.Swallowing and speaking were normal,without salivation or cheek biting.There was no obvious complication in the donor site.Conclusion:The success rate is high,the morphology and function may recover well after reconstruction of through and through cheek defects with folded free anterolateral thigh flap following resection of buccal carinoma.
