腭裂手术患者口角损伤情况分析及防护初探

摘　　要：目的:研究常规腭裂手术患者口角损伤情况、影响因素及预防方法。方法:观测记录500例常规行腭咽部手术的腭裂患者及10例使用口角保护器的腭裂患者术后口角损伤情况,将损伤程度分为无损伤、轻度、中度及重度损伤四级。结果:500例常规手术后396例(79.2%)患者出现口角损伤,其中205例(41%)患者出现中重度损伤。年龄小于3岁组患者口角中重度损伤发生率显著高于大年龄组(P<0.05)。口角损伤发生率和损伤程度与口裂大小、手术类型、手术医师无明显相关(P>0.05)。10例使用口角保护器的腭裂患者术后无口角损伤发生。结论:腭裂手术患者发生口角损伤十分普遍,低龄患者更为严重,需要采取保护措施。自行研发的口角保护器可有效预防口角损伤发生。Objective:To analyze the commissural lip injury and influencing factors in the patients undergoing conventional cleft palate surgery,and to explore a innovative preventive method.Methods:500 patients with cleft palate underwent routine palatopharyngeal surgery and 10 patients used corner protector in the operation were included.The degree of injury was divided into 4 levels:No injury,mild injury,moderate injury and severe injury.Results:396 patients(79.2%)had commissural lip injury and 205(41%)had moderate to severe injury after routine operation.The incidence of moderate and severe injury in the age group below 3 years old was significantly higher than that in the older age groups(P<0.05).There was no significant difference of the incidence rate and degree of commissural lip injury among the sizes of oral fissure,the types of operation and the surgeons(P>0.05).None of the 10 patients who used commissural lip protector had lip injury after operation.Conclusion:It is common for the patients with cleft palate to suffer from commissural lip injury after surgery,especially in young patients,and protective measures should be taken.The self-developed commissural lip protector can effectively prevent the lip injury.
