不同根管充填方式在牙体牙髓病患者根管治疗中的应用效果比较

摘　　要：目的:比较不同根管充填方式在牙体牙髓病患者根管治疗中的应用效果。方法:回顾性分析2022年1—12月该院收治的120例牙体牙髓病患者的临床资料,根据不同填充方法分为A组(AH-plus糊剂热牙胶垂直加压充填)、B组(AH-plus糊剂冷牙胶侧压充填)、C组(iRootSP糊剂单尖法充填)各40例。比较三组治疗前后咀嚼功能、牙周相关指标水平、疼痛程度[视觉模拟评分法(VAS)]评分、炎性因子[肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、超敏C反应蛋白(hs-CRP)、白细胞计数]水平和并发症发生率。结果:治疗后,三组咬合力、咀嚼效率均高于治疗前,且C组高于A组、B组,差异有统计学意义(P<0.05);三组牙龈指数、出血指数、松动度均低于治疗前,且C组低于A组、B组,差异有统计学意义(P<0.05);三组VAS评分均低于治疗前,且C组低于A组、B组,差异有统计学意义(P<0.05);三组TNF-α、IL-6、hs-CRP水平及白细胞计数均低于治疗前,且C组低于A组、B组,差异有统计学意义(P<0.05);A组与B组并发症发生率比较,差异无统计学意义(P>0.05);C组并发症发生率低于A组、B组,差异有统计学意义(P<0.05)。结论:iRootSP糊剂单尖法充填治疗牙体牙髓病患者可提高咬合力和咀嚼效率,降低牙周相关指标水平、VAS评分和并发症发生率,其效果优于AH-plus糊剂热牙胶垂直加压充填或AH-plus糊剂冷牙胶侧压充填治疗。Objective:To compare application effects of different root canal filling methods in root canal treatment of patients with dental pulp disease.Methods:The clinical data of 120 patients with dental pulp disease admitted to this hospital from January to December 2022 were retrospectively analyzed.According to different filling methods,they were divided into group A(AH-plus paste warm gutta-percha vertical condensation filling),group B(AH-plus paste cold gutta-percha lateral condensation filling),and group C(iRootSP paste single-cone obturation),40 cases in each group.The masticatory function,the periodontal related index levels,the pain degree[visual analogue scale(VAS)]score,the inflammatory factor levels[tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),high-sensitivity C-reactive protein(hs-CRP),white blood cell count],and the incidence of complications were compared among the three groups before and after the treatment.Results:After the treatment,the bite force and the masticatory efficiency of the three groups were higher than those before the treatment,those of group C were higher than those of group A and group B,and the differences were statistically significant(P<0.05).The levels of gingival index,bleeding index and looseness of the three groups were lower than those before the treatment,those of group C were lower than those of group A and group B,and the differences were statistically significant(P<0.05).The VAS scores of the three groups were lower than those before the treatment,that of the C group was lower than those of the group A and the group B,and the difference was statistically significant(P<0.05).The levels of TNF-α,IL-6,hs-CRP and white blood cell count in the three groups were lower than those before the treatment,those in group C were lower than those in group A and group B,and the differences were statistically significant(P<0.05).There was no significant difference in the incidence of complications between group A and group B(P>0.05).The incidence of complications in group C was lower
