摘 要:目的针对颅脑损伤患者,探讨分析采用以循证为指导的人工气道集束化护理干预对呼吸机相关性肺炎预防的作用。方法从三明市第二医院2020年1月至2022年12月诊疗的颅脑损伤患者中抽取64例进行回顾性分析,根据护理方式的不同将其分为对照组(n=32,常规护理)和观察组(n=32,常规护理加以循证为指导的人工气道集束化护理)。对比两组护理前后的血气[动脉血氧分压(PaO_(2))、动脉血氧饱和度(SaO_(2))、动脉血二氧化碳分压(PaCO_(2))]指标水平,并统计两组临床指标(ICU入住时间、住院时间、机械通气时间)、呼吸机相关性肺炎发生率。结果观察组护理后的PaO_(2)、SaO_(2)水平较护理前升高且高于对照组,且PaCO_(2)水平较护理前下降且低于对照组(P<0.05);观察组ICU入住时间、住院时间、机械通气时间均短于对照组(P<0.05);观察组与对照组呼吸机相关性肺炎发生率分别为6.25%、31.25%,观察组更低(P<0.05)。结论在颅脑损伤患者中实施以循证为指导的人工气道集束化护理干预,能够改善其血气指标,缩短住院时间,有效预防呼吸机相关性肺炎。Objective To explore and analyze the effect of evidence-based artificial airway cluster nursing intervention on the prevention of ventilators associated pneumonia in patients with craniocerebral injury.Methods A total of 64 cases of craniocerebral injury patients diagnosed and treated in Sanming Second Hospital Sanming Second Hospital from January 2020 to December 2022 were retrospectively analyzed,and divided into control group(n=32,routine nursing)and observation group(n=32,routine nursing followed by evidence-based artificial airway cluster nursing)according to different nursing methods.The blood gas[PaO_(2),SaO_(2),PaCO_(2)]indexes before and after care were compared between the two groups,and the clinical indexes(ICU stay time,hospital stay,mechanical ventilation time)and the incidence of ventilators associated pneumonia were analyzed.Results The levels of PaO_(2) and SaO_(2) in the observation group after nursing were higher and higher than those in the control group,and the level of PaCO_(2) was lower and lower than that in the control group(P<0.05).The ICU stay time,hospital stay time and mechanical ventilation time in observation group were shorter than those in control group(P<0.05).The incidence of ventilator-associated pneumonia in the observation group and the control group was 6.25%and 31.25%,respectively,and the incidence was lower in the observation group(P<0.05).Conclusions The implementation of evidence-based artificial airway cluster nursing intervention in patients with craniocerebral injury can improve blood gas indexes,shorten hospital stay and effectively prevent ventilators associated pneumonia.
