摘  要:目的研究Er,Cr:YSGG激光与差异性冲洗剂预处理离体牙桩道对纤维桩粘接强度的效果。方法采集因正畸需求拔除的单根前磨牙120颗开展研究。将其以随机数字表法分作A、B、C、D、E、F组,每组20颗。所有离体牙均实施体外根管治疗以及桩道预备,随后A、B、C组均开展非激光离体牙桩道预处理,D、E、F组均开展激光离体牙桩道预处理。A、D组冲洗剂选择蒸馏水,B、E组冲洗剂选择1%次氯酸钠(NaCLO)联合17%乙二胺四乙酸(EDTA),C、F组冲洗剂选择EDTA联合MTAD。比较6组纤维桩粘接强度、电镜微孔隙面积,桩道内电镜检查结果,根管壁牙本质表面钙磷含量。结果D组纤维桩拉伸强度以及电镜微孔隙面积高于A组,E组纤维桩拉伸强度以及电镜微孔隙面积高于B组,F组纤维桩拉伸强度以及电镜微孔隙面积高于C组,差异均有统计学意义(P<0.05);且A、B、C组纤维桩粘接强度、电镜微孔隙面积呈逐渐提升趋势(P<0.05);D、E、F组纤维桩粘接强度、电镜微孔隙面积亦呈逐渐提升趋势(P<0.05)。A组牙本质表面存在明显碎屑,且牙本质小管被彻底堵塞;B组牙本质表面存在少量碎屑,牙本质小管少数被堵塞;C组牙本质表明存在明显碎屑,且牙本质小管部分被堵塞;D组牙本质表面存在明显碎屑,牙本质小管堵塞明显;E组牙本质表面少量碎屑,牙本质小管无堵塞;F组牙本质表面存在少量碎屑,牙本质小管存在部分堵塞。D、E、F组钙、磷含量均高于A、B、C组(P<0.05)。结论Er,Cr:YSGG激光联合冲洗剂预处理离体牙桩道可显著提高纤维桩粘接强度,且冲洗剂选用EDTA联合MTAD的效果最为理想。Objective To study the effect of erbium,chromium:yttrium-scandium-gallium-garnet(Er,Cr:YSGG)laser and different detergent pretreatmentsofisolated dental post on the adhesion strength of fiber post.Methods A total of 120 single premolars extracted due to orthodontic requirements were collected.They were randomly divided into group A-F,with 20 teeth in each group.All the isolated teeth were treated with external root canal therapy and post canal preparation.Then,non-laser tooth impact pretreatment,and laser tooth impact pretreatment were performed in group A-C,and group D-F,respectively.Distilled water,1%sodium hypochlorite(NaCLO)combined with 17%ethylenediamine tetraacetic acid(EDTA),and EDTA combined with mixture of Doxycycline citric acid and detergent(MTAD)were used as the detergent in group A/D,B/E and C/F,respectively.The bond strength,micropore area,electron microscopic examination results in the post canal,and the content of calcium and phosphorus on the dentin surface of the root canal wall were compared among the six groups.Results Significantly higher tensile strength and micropore area under electron microscope were detected in group D versus group A,group E versus group B,and group F versus group C(P<0.05).The bond strength of the fiber piles and micropore area under electron microscope in group A,B and C gradually increased,and which in group D,E and F also gradually increased(both P<0.05).In group A,there were obvious detritus on the dentin surface,and the dentin tubules were completely blocked.In group B,there was a small amount of debris on the dentin surface,and a few dentin tubules were blocked.Dentin in group C showed obvious detritus and partially blocked dentin tubules.In group D,there were obvious detritus on the dentin surface and obvious occlusion of dentin tubules.In group E,there was little detritus on the dentin surface and no occlusion of dentin tubules.In group F,there was a small amount of debris on the dentin surface and partial occlusion of dentin tubules.The contents of calcium and
